穴位埋线结合艾灸治疗原发性痛经患者对子宫动脉血流动力学和VAS评分的影响

摘　　要：目的:分析穴位埋线结合艾灸治疗原发性痛经(PD)患者对子宫动脉血流动力学和疼痛视觉模拟评分(VAS)的影响。方法:选取2016年1月—2017年12月上海市第二人民医院收治的PD患者60例,随机数字表法分为治疗组、对照组各30例,对照组予以常规西药及艾灸治疗,治疗组在此基础上予以聚对二氧环己酮(PPDO)进行穴位埋线治疗,连续治疗3个月经周期,比较两组临床疗效、子宫动脉血流动力学、VAS评分、血清指标及不良反应。结果:治疗组治疗总有效率90.00%(27/30),高于对照组的66.67%(20/30)(P<0.05);治疗后治疗组阻力指数(RI)和搏动指数(PI)、收缩末期峰值/舒张末期峰值(S/D)低于对照组,而平均血流速度(Vm)高于对照组(P<0.05);治疗2个月、治疗3个月、治疗后1个月治疗组VAS评分低于对照组(P<0.05);治疗后治疗组血清前列腺素E2(PGE2)、β内啡肽(β-EP)高于对照组,而前列腺素F2α(PGF2α)低于对照组(P<0.05);两组均未见明显的局部或全身不良反应。结论:穴位埋线结合艾灸治疗PD有较好疗效,可明显改善子宫动脉血流动力学,减轻疼痛,安全性可靠,值得在临床推广实践。Objective:To analyze the effects of acupoint catgut embedding combined with moxibustion on uterine artery hemodynamics and VAS scores in patients with primary dysmenorrhea(PD).Methods:A total of 60 patients with PD were randomly divided into the treatment group and the control group,with 30 patients in each group.The control group was treated with routine western medication and moxibustion;on which basis,the treatment group was also treated with polydioxanone(PPDO)for acupoint catgut embedding.Both groups were continuously treated for 3 menstrual cycles.The clinical curative effect,uterine artery hemodynamics,VAS scores,serum indexes and adverse reactions were compared between the two groups.Results:The effective rate of the treatment group was 90.00%(27/30),which was significantly higher than 66.67%(20/30)of the control group(P<0.05).Resistance index(RI),pulsatility index(PI)and peak systolic velocity/end diastolic velocity(S/D)were lower,and mean flow velocity(Vm)was higher in the treatment group than those in control group(P<0.05).VAS scores were lower in treatment group than those in the control group after 2 months and 3 months of the treatment,and at 1 month after the treatment(P<0.05).The serum levels of prostaglandin E2(PGE2)andβ-endorphin(β-EP)were higher,whereas the level of prostaglandin F2α(PGF2α)was lower in the treatment group than those in the control group(P<0.05).There were no significant local or systemic adverse reactions in the two groups.Conclusion:Acupoint catgut embedding combined with moxibustion has a significant effect on PD,which can significantly improve uterine artery hemodynamics and relieve pain,with good safety and reliability.The therapy is worthy of clinical promotion.
