基于关联规则探索明清时期针灸治疗面瘫选穴规律研究

摘　　要：目的:探析明清时期针灸治疗面瘫的腧穴选用规律。方法:利用古代针灸处方管理信息系统,检索明清时期针灸治疗面瘫处方193首,获取腧穴和经脉的选用频数,发现主要的腧穴和经脉;利用SPSS Modeler 18.0统计软件,发现腧穴之间关联链接关系,挖掘腧穴之间关联规则,并采用规则约简方法,获取有价值的关联规则。结果:明清针灸治疗面瘫的主要腧穴包括16个,其中频数最高的8个腧穴是地仓、颊车、听会、合谷、水沟、承浆、太渊和百会,频数最高的6条经脉是足阳明胃经、足少阳胆经、手阳明大肠经、督脉、手太阴肺经和任脉;关联链接分析发现12个中等以上强度的腧穴关联链接,其中关联链接度最强的腧穴对是“地仓+颊车”;关联规则挖掘发现19条满足参数阈值和评估测量要求的规则,其中规则支持度≥10的腧穴关联规则包括5条,分别为“水沟+颊车→合谷”“合谷+地仓+颊车→水沟”“承浆+地仓→合谷”“水沟+地仓→合谷”和“水沟+地仓+颊车→合谷”;并经规则约简处理,选定腧穴关联规则2条,分别为“承浆+地仓→合谷”和“水沟+地仓+颊车→合谷”。结论:明清时期针灸治疗面瘫的选穴,采用分部取穴、循经取穴原则,注重循经远取、远近配合,腧穴的选用及其配伍特点为现代针灸临床治疗面瘫提供了用穴规律参考;规则约简方法有助于获取更具价值的关联规则。Objective:To explore the laws of acupoints selection for facial paralysis treated by acupuncture and moxibustion in Ming and Qing dynasties.Methods:By using the ancient acupuncture and moxibustion prescription management information system,193 prescriptions treating facial paralysis in Ming and Qing dynasties were retrieved to obtain frequency of acupoints and meridians,thus to find principal acupoints and meridians.On this basis,association links were obtained and association rules between acupoints were mined by using SPSS Modeler 18.0 software and the rule reduction method.Results:For acupuncture and moxibustion treatment of facial paralysis in Ming and Qing dynasties,there were 16 principal acupoints.8 acupoints with the highest frequency were Dicang(ST4),Jiache(ST6),Tinghui(GB2),Hegu(LI4),Shuigou(DU26),Chengjiang(RN24),Taiyuan(LU9)and Baihui(DU20).6 meridians with the highest frequency were Stomach Meridian of Foot-Yangming,Gall Bladder Meridian of Foot-Shaoyang,Large Intestine Meridian of Hand-Yangming,Du Meridian,Lung Meridian of Hand-Taiyin and Ren Meridian.By association link analysis,12 acupoint associations of moderate or above strength were obtained.Among them,the points pair of ST4 plus ST6 was the most highly correlated.By mining association rules,19 rules were found which met parameter thresholds and evaluation measurement requirements;among them,5 rules met the requirement of rule support being 10 or greater,which were DU26+ST6→LI4,LI4+ST4+ST6→DU26,RN24+ST4→LI4,DU26+ST4→LI4 and DU26+ST4+ST6→LI4.Finally,by using the rule reduction method,two rules were selected,which were RN24+ST4→LI4 and DU26+ST4+ST6→LI4.Conclusion:For acupuncture and moxibustion treatment of facial paralysis in Ming and Qing dynasties,the acupoints selection was based on body parts and along the meridians,and it paid attention to the combination of local and distal acupoints.The features of acupoints selection and compatibility could provide some references for the use of acupoints in the clinical treatment of facia
