摘 要:目的:观察电针对退变腰椎间盘模型兔脊髓背角中甲硫脑啡肽(M-ENK)、β-内啡肽及背根神经节中Netrin-1含量及相关性的影响,探讨电针改善盘源性腰痛的可能作用机制。方法:随机选取4只新西兰大白兔作为正常组,并随机选取12只实验兔予以轴向椎体加压诱导退变腰椎间盘模型,造模成功后随机分为模型组、电针组和假电针组,每组各4只,另随机选取4只不予轴向椎体加压处理作为假模型组。电针组取L_(4)、L_(5)双侧夹脊穴,连接电针仪治疗,疏密波,频率2/15 Hz,强度1~2 mA,20 min/次,1次/d,一疗程6次,共治疗4个疗程;假电针组针刺L_(4)、L_(5)夹脊穴夹持电针但不通电,其余操作同电针组。治疗结束后,Western blot法检测各组实验兔L_(2)脊髓背角中M-ENK、β-内啡肽以及背根神经节中Netrin-1蛋白表达,直线相关分析法分析M-ENK、β-内啡肽的表达与Netrin-1之间的相关性。结果:与正常组比较,模型组退变腰椎间盘相应背根神经节中Netrin-1蛋白表达明显下降;脊髓背角中M-ENK、β-内啡肽蛋白表达明显增加(P<0.05);与模型组比较,电针组Netrin-1、M-ENK及β-内啡肽蛋白表达明显增加(P<0.05);与电针组比较,假电针组Netrin-1、M-ENK及β-内啡肽蛋白明显减少(P<0.05)。直线相关分析法Netrin-1蛋白含量与M-ENK、β-内啡肽之间均存在正相关性(R^(2)=0.66、R^(2)=0.51)。结论:电针可上调背根神经节中Netrin-1的表达,改善盘源性疼痛,其作用可能与上调脊髓背角中M-ENK、β-内啡肽的表达有关。Objective:To observe the effects of electro-acupuncture(EA)on the contents of methionine enkephalin(M-ENK),β-endorphin(β-EP)and Netrin-1 in dorsal root ganglion(DRG)of degenerative lumbar intervertebral disc of the model rabbits,and to explore the possible mechanism of EA in improving discogenic lower-back pain.Methods:Four New Zealand white rabbits were randomly selected as the normal control,and 12 experimental rabbits were randomly selected to induce degenerative lumbar intervertebral disc model by axial vertebral compression.After successful modeling,these rabbits were randomly divided into the model group,the EA group and the sham EA group,with 4 rabbits in each group.Another 4 rabbits were randomly selected as the sham model group without axial vertebral compression.In the EA group,L_(4) and L_(5) Jiaji points were selected for EA treatment for four courses.In the sham EA group,L_(4) and L_(5) Jiaji points were selected for EA but without electricity,and the rest operations were the same as those in the EA group.After the treatment,Western blot was used to detect the expressions of M-ENK andβ-EP in L_(2) spinal dorsal horn and Netrin-1 in DRG.Linear correlation analysis was used to analyze the correlation between the expressions of M-ENK,β-EP and Netrin-1.Results:Compared to those in the normal group,the protein expressions of Netrin-1 in the corresponding DRG of degenerative lumbar intervertebral disc was significantly decreased,and the protein expressions of M-ENK andβ-EP in the spinal dorsal horn were significantly increased in the model group(P<0.05).Compared to those in the model group,the protein expressions of Netrin-1,M-ENK andβ-EP were significantly increased in the EA group(P<0.05).Compared to those in the EA group,the protein expressions of Netrin-1,M-ENK andβ-EP were significantly decreased in the sham EA group(P<0.05).Linear correlation analysis showed that Netrin-1 protein content was positively correlated with M-ENK andβ-EP(R^(2)=0.66,R^(2)=0.51).Conclusion:EA can up-regulate the expr
