透明质酸钠联合雌激素在人工流产术后的应用效果研究

摘　　要：目的探究透明质酸钠联合雌激素在人工流产术后患者中的应用效果。方法选取2019年6月—2020年12月两次及两次以上人工流产的早孕产妇156例为研究对象,将其根据干预方法的不同分为A组(常规干预组)35例、B组(透明质酸钠组)42例、C组(戊酸雌二醇组)41例和D组(透明质酸钠联合戊酸雌二醇组)38例。比较四组的阴道出血时间、子宫内膜厚度、月经复潮时间、宫腔粘连发生率及宫颈分泌物前炎因子、抗炎因子水平。结果D组的阴道出血时间及月经复潮时间显著短于A组、B组及C组,B组及C组的阴道出血时间及则显著短于A组,D组的子宫内膜厚度显著高于A组、B组及C组,B组及C组则显著高于A组(P<0.05);C组及D组的宫腔粘连发生率显著低于A组(P<0.05);D组的宫颈分泌物前炎因子显著低于A组、B组及C组,B组及C组显著低于A组(P<0.05);D组的宫颈分泌物抗炎因子水平显著高于A组、B组及C组,B组及C组显著高于A组(P<0.05)。结论透明质酸钠联合雌激素可有效降低人工流产术后的宫腔粘连,对于宫颈分泌物炎性因子的表达也有相对较好的作用,在人工流产患者中的应用价值较高。Objective To investigate the effect of sodium hyaluronate combined with estrogen in patients after induced abortion.Methods A total of 156 early pregnant women with two or more abortions from June 2019 to December 2020 were chosen as the study object,and they were divided into group A(routine intervention group)35 cases,group B(sodium hyaluronate group)42 cases,group C(estradiol valerate group)41 cases and group D(sodium hyaluronate combined with estradiol valerate group)38 cases according to the differences of intervention methods.Then the vaginal bleeding time,duration of abdominal pain,endometrial thickness,menstruation time,menstrual volume,intrauterine adhesions rates and cervical secretions proinflammatory and anti-inflammatory factors of four groups weer compared.Results The vaginal bleeding time and menstrual re-fluid time in group D were significantly shorter than those in groups A,B and C,the vaginal bleeding time in groups B and C was significantly shorter than that in group A;the thickness of the endometrium in group D was significantly higher than that of group A,B and C,groups B and C were significantly higher than group A(P<0.05).The incidence of intrauterine adhesions in groups C and D was significantly lower than that in group A(P<0.05).The proinflammatory factors of cervical secretion in group D were significantly lower than those in group A,group B and group C,group B and group C were significantly lower than that in group A(P<0.05).The levels of anti-inflammatory factors in cervical secretions of group D were significantly higher than those of group A,group B and group C,group B and group C were significantly lower higher than that of group A(P<0.05).Conclusion The mechanism of sodium hyaluronate combined with estrogen can effectively reduce the intrauterine adhesions of patients after induced abortion,and its influence role for expression of cervical secretion inflammatory factors are relatively better,so its application value in the patients with induced abortion is higher.
