丙泊酚复合瑞芬太尼麻醉在腹腔镜卵巢囊肿切除术中的效果及安全性

摘　　要：目的探讨腹腔镜卵巢囊肿切除术患者应用丙泊酚复合瑞芬太尼麻醉的效果及安全性。方法选取72例行腹腔镜卵巢囊肿切除术治疗的卵巢囊肿患者,按照随机数字表法分对照组与研究组,每组36例。对照组予以丙泊酚复合芬太尼麻醉诱导,研究组予以丙泊酚复合瑞芬太尼麻醉诱导。比较两组麻醉效果及安全性。结果两组麻醉前、麻醉诱导后、插管时、气腹5 min后、停药时心率及平均动脉压均存在一定波动,但同一时间点组间比较差异无统计学意义(P>0.05)。研究组麻醉诱导时间、苏醒时间及定向力恢复时间均明显短于对照组,差异有统计学意义(P<0.05)。研究组拔管时、离开手术室时、拔管后1h术后意识状态(OAAS)评分均明显高于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率为5.56%,明显低于对照组的22.22%,差异有统计学意义(P<0.05)。结论丙泊酚复合瑞芬太尼用于腹腔镜卵巢囊肿切除术麻醉中不仅可缩短麻醉诱导时间,还能促进术后恢复,改善患者术后意识状态,且安全性较高。Objective To investigate the effect and safety of propofol combined with remifentanil anesthesia in patients undergoing laparoscopic ovarian cystectomy.Methods 72 patients with ovarian cyst undergoing laparoscopic surgery were selected and divided into control group and study group according to random number table method,with 36 cases in each group.The control group was given propofol combined with fentanyl anesthesia,the study group was given propofol combined with remifentanil general anesthesia.The anesthetic effect and safety were compared between the two groups.Results There were some fluctuations in heart rate and mean arterial pressure before anesthesia,after anesthesia induction,during intubation,5 min after pneumoperitoneum,and when drug withdrawal,but there was no statistical significance between the two groups at the same time point(P>0.05).The anesthesia induction time,awakening time and directional force recovery time in the study group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).The OAAS score of the study group was significantly higher than that of the control group during extubation,after leaving the operating room and 1 h after extubation,and the difference was statistically significant(P<0.05).The incidence of adr in the study group was 5.56%,which was significantly lower than that in the control group 22.22%,and the difierence was statistically significant(P<0.05).Conclusion The use of propofol combined with remifentanil in anesthesia during laparoscopic ovarian cystectomy can not only shorten the induction time of anesthesia,but also promote postoperative recovery,improve postoperative consciousness of patients,and has high safety.
