经脐单孔腹腔镜术在输卵管妊娠中的应用研究

摘　　要：目的探讨经脐单孔腹腔镜术治疗输卵管妊娠患者的应用效果。方法选取80例输卵管妊娠患者,依据患者意愿将其分为研究组与对照组,每组40例。研究组采用经脐单孔腹腔镜术治疗,对照组采用传统三孔腹腔镜术治疗。比较两组手术指标、住院时间、住院费用、术后并发症发生情况、术后疼痛情况及满意度评分。结果研究组手术时间明显长于对照组,差异有统计学意义(P<0.05),但两组患者术中出血量、术后肠道恢复排气时间、尿管拔除恢复排尿时间、住院时间及住院费用比较,差异无统计学意义(P>0.05)。研究组术后并发症发生率为2.5%,明显低于对照组的17.5%,差异有统计学意义(P<0.05)。研究组视觉模拟评分法(VAS)评分(2.86±0.32)分低于对照组(3.65±1.03)分,患者满意度评分(97.56±1.98)分高于对照组(82.63±1.84)分,差异具有统计学意义(P<0.05)。结论输卵管妊娠患者采用经脐单孔腹腔镜术治疗效果显著,可以有效减轻患者疼痛,降低并发症发生率,患者满意度较高,值得临床推广与应用。Objective To investigate the effect of single port laparoscopy through umbilicus in treating tubal pregnancy.Methods 80 patients with tubal pregnancy were selected and divided into study group and control group according to their wishes,with 40 cases in each group.The study group was treated with single-port transumbilical laparoscopy,while the control group was treated with traditional three-port laparoscopy.Operation indicators,length of stay,hospitalization costs,postoperative complications,postoperative pain and satisfaction scores were compared between the two groups.Results The operative time of the study group was significantly longer than that of the control group,and the difference was statistically significant(P<0.05),but there were no significant differences in intraoperative blood loss,postoperative intestinal recovery time,recovery time of urination after catheter removal,length of hospital stay and hospitalization cost between the two groups(P>0.05).The incidence of postoperative complications in the study group was 2.5%,which was significantly lower than that in the control group(17.5%),with statistical significance(P<0.05).The visual analog scale(VAS)score of the study group(2.86±0.32)was lower than that of the control group(3.65±1.03),and the patient satisfaction score of the study group(97.56±1.98)was higher than that of the control group(82.63±1.84),with statistically significant differences(P<0.05).Conclusion Transumbilical single-port laparoscopic treatment for tubal pregnancy has significant effects,which can effectively relieve pain in patients,reduce the incidence of complications,and have high patient satisfaction,which is worthy of clinical promotion and application.
