摘　　要：目的探讨产前超声诊断胎儿大动脉转位(TGA)的临床价值。方法回顾性分析11例产前超声诊断为TGA胎儿的临床资料,总结相关切面的超声特征和诊断结果并与产后超声心动图或尸检病理结果进行比较,分析误诊、漏诊原因。结果11例TGA胎儿中,10例经产后超声心动图或尸检病理结果证实,其中单纯完全型TGA 4例,复合完全型TGA 4例(1例合并室间隔缺损和主动脉缩窄,2例合并心内膜垫缺损和肺动脉狭窄,1例合并室间隔缺损),矫正型TGA 2例;另1例为误诊,经产后超声心动图证实为室间隔缺损合并左心发育不良。产前漏诊1例。结论TGA的产前超声表现具有一定特征性,需重点关注左、右心室流出道切面,结合大动脉追踪扫查,避免漏诊、误诊。Objective To explore the clinical value of prenatal ultrasound in the diagnosis of fetal transposition of great arteries(TGA).Methods The clinical data of 11 cases with TGA fetuses diagnosed by prenatal ultrasound were analyzed retrospectively.The ultrasonic characteristics and diagnostic results of relevant cross sections were summarized,and the results were compared with those of postpartum echocardiography or autopsy pathology.The causes of misdiagnosis and missed diagnosis were analyzed.Results Of the 11 cases diagnosed with TGA,10 cases were confirmed by postpartum echocardiography or autopsy pathology,including 4 cases of simple complete TGA,4 cases of compound complete TGA(one case with ventricular septal defect and coarctation of aorta,2 cases with endocardial cushions defect and pulmonary artery stenosis,and one case with ventricular septal defect),and 2 cases of corrected TGA.One misdiagonsed case with ventricular septal defect and left ventricular dysplasia was confirmed by postpartum echocardiography.One case was missed prenatal diagnosis.Conclusion The prenatal ultrasound images of TGA have certain characteristics.It is necessary to focus on the section of the left and right ventricular outflow tract combined with aorta tracking and scanning to avoid the misdiagnosis and missed diagnosis.
