人肝细胞癌HepG2细胞荷瘤裸鼠^(18)F⁃乙基胆碱、^(11)C⁃胆碱、^(18)F⁃FDG生物分布和PET肿瘤显像

摘  要:目的单一^(18)F⁃FDG PET显像对高分化HCC诊断敏感低,因而探讨荷人肝细胞癌HepG2细胞裸鼠中^(18)F⁃乙基胆碱、^(11)C⁃胆碱、^(18)F⁃FDG的活体生物学分布以及^(18)F⁃乙基胆碱(^(18)F⁃FECh)、^(11)C⁃胆碱、^(18)F⁃FDG microPET显像检测荷瘤鼠肿瘤的价值。方法荷人肝癌细胞HepG2裸鼠3只,行^(18)F⁃FECh、^(11)C⁃胆碱、^(18)F⁃FDG microPET动态扫描,分析不同时间点3种显像剂在脑、心腔、肝、肺、肾皮质、肌肉、肿瘤的组织摄取值(%ID/g)和瘤/肝比、瘤/肌比。结果^(18)F⁃FECh、11 C⁃胆碱与^(18)F⁃FDG的生物分布有差异,肝、肾的^(18)F⁃FECh、^(11)C⁃胆碱摄取高,在其他正常组织摄取低。肾、心、肝和脑组织^(18)F⁃FDG摄取高。不同时间点^(18)F⁃FECh、^(11)C⁃胆碱瘤/肝比和瘤/肌比平稳,而^(18)F⁃FDG变化大。不同时间点^(18)F⁃FDG瘤/肝比和20~60 min时间点^(18)F⁃FDG瘤/肌比明显高于^(18)F⁃FECh和^(11)C⁃胆碱。5、10、20、30 min ^(18)F⁃FECh瘤/肝比与11 C⁃胆碱差异无统计学意义(P>0􀆰05),但在20、30 min ^(18)F⁃FECh瘤/肌比(3.70±1.27、3.85±1.44)显著高于11 C⁃胆碱(1.99±1.31、0.00±0.00),差异有统计学意义(P<0􀆰05)。结论荷瘤鼠中肾和肝是胆碱代谢的主要器官,^(18)F⁃FECh、^(11)C⁃胆碱的生物学分布理想。与^(11)C⁃胆碱相比,^(18)F⁃FECh更容易观察肿瘤,因此可选用^(18)F⁃FDG和^(18)F⁃FECh双探针PET显像模式,提高肝细胞癌诊断的准确性。Objective Because of the low diagnostic sensitivity of single ^(18)F⁃FDG PET imaging in high differentiated human hepatocellular carcinoma,the aim of this study is to analyze in vivo biological distribution of ^(18)F⁃FECh,^(11)C⁃choline and ^(18)F⁃FDG in nude mice bearing human hepatocellular carcinoma HepG2 cells,and to explore the value of tumor detection with ^(18)F⁃FECh,^(11)C⁃cho⁃line and ^(18)F⁃FDG microPET imaging􀆰Methods ^(18)F⁃FECh,^(11)C⁃choline and ^(18)F⁃FDG microPET dynamic imaging were studied in tumor⁃bearing mice(n=3).The uptake value(%ID/g)of brain,heart(heart cavity),liver,lung,kidney(kidney cortex),muscle,tumor lesion and the uptake ratios of tumor/liver,tumor/muscle were analyzed at different time⁃points􀆰Results Biodistri⁃butions of ^(18)F⁃FECh,^(11)C⁃choline and ^(18)F⁃FDG were different.High uptake of ^(18)F⁃FECh,^(11)C⁃choline in liver and kidney,low uptake in other normal tissue were obtained.High uptake of ^(18)F⁃FDG in kidney,heart,liver and brain were observed.Tumor/liver ratios,tumor/muscle ratios of ^(18)F⁃FECh and ^(11)C⁃choline were stabilized at different time⁃points,but those of ^(18)F⁃FDG was significantly changed.Tumor/liver ratios of ^(18)F⁃FDG at different time⁃points and tumor/muscle ratios of ^(18)F⁃FDG within the range of 20⁃60 min were higher than those of ^(18)F⁃FECh,^(11)C⁃choline.No significant difference of tumor/liver ratios at 5 min,10 min,20 min,30 min were observed between ^(18)F⁃FECh and ^(11)C⁃choline,however,tumor/muscle ratios of ^(18)F⁃FECh at 20 min,30 min was significantly higher compared with ^(11)C⁃choline􀆰Conclusion Kidney and liver are the main organs of choline metabolism,^(18)F⁃FECh and ^(11)C⁃choline have a favorable biodistribution in tumor⁃bearing mice.The tumor was easier to be detected by ^(18)F⁃FECh compared with ^(11)C⁃choline,so dual⁃tracer PET imaging modality with ^(18)F⁃FDG and ^(18)F⁃FECh can serve to improve the diagnosis accuracy of hepatocellular
