摘  要:为了解广东地区羊源凝固酶阴性葡萄球菌(CNS)的耐药情况,本研究于2018年~2020年采集了279份具有呼吸道症状羊的鼻拭子,进行了细菌的分离鉴定、小鼠的致病性试验、药敏试验和耐药基因的检测,并分析了耐药表型与耐药基因的相关性。结果显示,共分离得到41株CNS,其中溶血葡萄球菌的检出率最高,为19.6%(9/41)。CNS对小鼠有一定的致病性,致死率为30%(3/10),其中脾脏和肺脏载菌量最多,且从死亡小鼠的脏器中分离到了接种的病原菌。所有CNS均对头孢吡肟敏感,对青霉素高度耐药,耐药菌株占75.6%(31/41),其次是对阿奇霉素、红霉素、苯唑西林和克林霉素耐药,耐药菌株分别为68.3%(28/41)、68.3%(28/41)、65.9%(27/41)和65.9%(27/41);多重耐药现象严重,其中耐6种抗生素和9种抗生素的菌株各占14.6%(6/41)。耐药基因检测结果显示,大环内酯类耐药基因ermC和msrA的检出率较高,分别为46.3%(19/41)和24.4%(10/41),其次为β-内酰胺类耐药基因mecA,检出率为31.7%(13/41);β-内酰胺类耐药基因TEM、SHV的检出率低,分别为7.3%(3/41)和4.9%(2/41)。与耐药表型符合率较高的耐药基因是ac(6)-Ib-cr和sul1,符合率均为85.4%,其次是qnrS,与耐药表型的符合率为80.5%。广东地区羊源CNS的耐药现象严重,耐药表型和耐药基因的相关性较高,临床上可使用喹诺酮类抗生素。本研究首次针对广东地区引起羊呼吸道疾病的CNS进行致病性和耐药性调查,揭示CNS的流行菌株和耐药水平,对加强本地区养殖场的管理和耐药监测,以及指导规范使用抗菌药物具有一定的意义。To understand the drug resistance of coagulase negative Staphylococcus(CNS)from sheep in Guangdong,279 nasal swabs were collected from sheep with respiratory symptoms during 2018-2020.The isolation and identification,pathogenicity in mice,drug sensitivity test and drug resistance gene detection were performed,and the correlation between drug resistance phenotype and possession of drug resistance genes was analyzed.The results showed that a total of 41 CNS strains were isolated,of which Staphylococcus haemolyticus had the highest detection rate,19.6%(9/41).CNS had certain pathogenicity to mice with a lethality rate of 30%(3/10),and the spleen and lungs contained the most bacterial,and the same pathogens have been isolated from the organs of dead mice;all CNS were sensitive to cefepime and highly resistant to penicillin,the resistant strains accounted for75.6%(31/41),followed by azithromycin,erythromycin,oxacillin and clindamycin,the resistant strains accounted for 68.3%(28/41),68.3%(28/41),65.9%(27/41)and 65.9%(27/41),respectively.The multi-drug resistance was serious,of which strains resistant to6 antibiotics and 9 antibiotics accounted for 14.6%(6/41),respectively.The results of drug resistance gene detection showed that the detection rates of macrolide resistance genes ermC and msrA were high,46.3%(19/41)and 24.4%(10/41),respectively,followed byβ-lactams drug resistance gene mecA,with a detection rate of 31.7%(13/41);the detection rate ofβ-lactam resistance genes TEM and SHV were low,7.3%(3/41)and 4.9%(2/41)respectively.The resistance genes with a higher coincidence rate with the drug resistance phenotype were ac(6)-Ib-cr and sul1,with a coincidence rate of 85.4%,followed by qnrS with a coincidence rate of80.5%.Conclusion:The drug resistance of sheep-origin CNS in Guangdong was serious,and the drug resistance phenotype was highly correlated with drug resistance genes.Quinolones antibiotics can be used clinically.This study is the first to investigate the pathogenicity and drug resistance of CNS causing respir
