摘  要:[目的]探讨茯苓四物汤对造影剂诱导的急性肾损伤(CI-AKI)模型大鼠肾功能及肾小管上皮细胞凋亡的影响。[方法]将40只SD大鼠随机分为4组:正常对照组、造影剂肾损伤模型组(简称“模型组”)、茯苓四物汤高剂量治疗组(简称“高剂量组”)及茯苓四物汤低剂量治疗组(简称“低剂量组”),每组10只。采用尾静脉注射吲哚美辛、左旋硝基精氨酸甲酯、碘海醇注射液制备急性肾损伤大鼠模型,低、高剂量组分别予茯苓四物汤(16.95 g/kg、33.9 g/kg)灌胃,正常对照组及模型组予生理盐水灌胃,灌胃容量均为20 ml/kg。尾静脉注射造影剂48 h后分别检测大鼠血液中血清肌酐(Scr)、尿素氮(BUN)的数值,再对大鼠肾脏制作病理切片及评估肾小管损伤程度、肾脏细胞凋亡及半胱氨酸天冬氨酸蛋白水解酶3(Caspase-3)表达情况。[结果]与正常对照组比较,模型组大鼠的Scr、BUN水平显著增加(P<0.01),而茯苓四物汤低、高剂量组的Scr、BUN水平较模型组显著下降(P<0.05);与正常对照组比较,模型组大鼠肾小管损伤评分显著升高(P<0.01),而茯苓四物汤低、高剂量组评分明显低于模型组(P<0.05);与正常对照组比较,模型组大鼠肾小管上皮细胞凋亡显著增加(P<0.01),茯苓四物汤低、高剂量组肾小管上皮细胞凋亡较模型组明显减少(P<0.01);与正常对照组比较,模型组大鼠Caspase-3的表达显著增加(P<0.01),特别是肾小管上皮区域,茯苓四物汤低、高剂量组Caspase-3的表达较模型组显著减少(P<0.01)。[结论]茯苓四物汤可以显著降低CI-AKI模型大鼠的Scr、BUN水平,减轻模型大鼠肾脏病理损伤程度及减少肾小管上皮细胞的凋亡,抑制Caspase-3的表达,提示茯苓四物汤具有肾保护作用。[Objective]To study the effect of Fuling Siwu Decoction on renal function and apoptosis of renal tubular epithelial cells in contrast-induced acute kidney injury(CI-AKI) model rats.[Methods]Forty SD rats were randomly divided into 4 groups: control group, contrast-induced kidney injury model group(model group), high-dose of Fuling Siwu Decoction group(high-dose group) and low-dose of Fuling Siwu Decoction group(low-dose group), with 10 rats in each group. A rat model of acute kidney injury was established by tail vein injection of indomethacin, L-nitroarginine methyl ester and iohexol injection by gavage, The high-dose and low-dose groups were given Fuling Siwu Decoction(16.95 g/kg, 33.9 g/kg) by gavage respectively. In the control group and model group were gavaged with normal saline, and the gavage volume was 20 ml/kg. The serum creatinine(Scr) and blood urea nitrogen(BUN) values in the blood of rats were detected 48 hours after the contrast agent was injected into the tail vein, and the pathological sections of the rat kidneys were made to evaluate the degree of renal tubular damage, renal cell apoptosis and cysteine expression of acid aspartate proteolytic enzyme 3(Caspase-3).[Results]Compared with the control group, the levels of Scr and BUN in the model group were significantly increased(P<0.01), while the levels of Scr and BUN in the low-dose and high-dose Fuling Siwu decoction groups were significantly decreased than those in the model group(P<0.05);compared with the control group, the renal tubular injury score in the model group was significantly increased(P<0.01);the scores in the low-dose and high-dose Fuling Siwu Decoction groups were significantly lower than those in the model group(P<0.05);the apoptosis of renal tubular epithelial cells in the model group was significantly increased(P<0.01),and the apoptosis of renal tubular epithelial cells in the low-dose and high-dose Fuling Siwu Decoction groups was significantly decreased compared with the model group(P<0.01).
