腹腔镜髂耻韧带悬吊术及其疗效的对照研究

摘　　要：目的比较腹腔镜髂耻韧带悬吊术与腹腔镜骶骨固定术治疗盆腔脏器脱垂(POP)及纠正轻中度压力性尿失禁(SUI)的疗效。方法选取2018年6月至2021年6月山东省淄博市中心医院收治的子宫脱垂患者,其中126例行腹腔镜下髂耻韧带悬吊术,42例行腹腔镜骶骨固定术。通过手术前后POP评价体系各指示点、盆底四维超声测量值的变化、盆底功能障碍问卷、尿失禁影响问卷简表评价两种手术方式术后POP改善情况。结果手术后POP-Q各指示点的位置得到显著改善(P<0.05)。术后盆底四维彩超测量值较术前明显减小,表明术后尿道高活动性降低,手术纠正POP同时治疗轻中度SUI,主观及客观指标均得到改善。结论腹腔镜髂耻韧带悬吊术治疗POP效果满意,同时治疗轻中度SUI疗效显著。Objective:To evaluate the efficacy of laparoscopic pectopexy in the treatment of pelvic organ prolapse(POP)with stress urinary incontinence(SUI).Methods:From June 2018 to June 2021,126 cases underwent laparoscopic pectopexy ligament suspension,and 42 cases underwent laparoscopic sacromobilization.Through the indication points of the POP evaluation system before and after surgery,the change of the pelvic floor four-dimensional ultrasound measurement,the pelvic floor dysfunction questionnaire,and the questionnaire summary of the impact of urinary incontinence,the improvement of pelvic organ prolapse after surgery was evaluated by two surgical methods.Results:Laparoscopic pectopexy can achieve satisfactory results,improved patients'quality of life,caused less damage to the body,and had fewer complications,had high subjective satisfaction and low recurrence rate.The subjective and objective indicators of SUI had been improved.Conclusion:Laparoscopic pectopexy is an effective treatment of uterine prolapse combined with mild and moderate stress urinary incontinence.
