低温等离子射频消融术治疗早期声门型喉癌对嗓音功能的影响

摘　　要：目的研究在支撑喉镜下应用低温等离子射频消融技术对早期声门型喉癌治疗后嗓音功能的影响。方法试验组纳入2019年1月至2020年3月牡丹江医学院附属红旗医院耳鼻咽喉头颈外科收治住院的早期声门型喉癌患者30例,在支撑喉镜下采用低温等离子射频消融技术切除癌灶,记录术前、术后1、3、6个月及1年行电子喉镜检查手术创面情况,并对主、客观嗓音情况进行长期随访评估;对照组采集同时间段体检的30名健康成年人的客观嗓音指标进行对比。结果术后1年内试验组患者未见癌灶复发,声带运动好,声门闭合不全,无明显并发症出现。术后3个月,嗓音主观评估指标显示嗓音仍处于中度异常,客观指标显示嗓音较术前更差;术后6个月,声带基本特征逐渐恢复;术后1年,声音嘶哑程度较术前有所改善,但无法恢复至正常人嗓音状态。结论经支撑喉镜下采用低温等离子射频消融技术对早期声门型喉癌的患者嗓音功能恢复有利,值得临床广泛应用。Objective To study the effect of low-temperature plasma radiofrequency ablation under suspension laryngoscope on voice function in the treatment of early glottic carcinoma.Methods A total of 30 patients with early glottic carcinoma who were admitted to the Department of Otorhinolaryngology Head and Neck Surgery,Hongqi Hospital Affiliated to Mudanjiang Medical University from January 2019 to March 2020 were included in the experimental group.And the removal of carcinoma foci by low-temperature plasma radiofrequency ablation under support laryngoscope was performed on patients in the experimental group.Before operation,1 month,3 months,6months and 1 year after operation,electronic laryngoscopy was performed to examine the operative wounds and long-term follow-up evaluation of the subjective and objective voices.30 healthy adults who underwent physical examination at the same period were included in the control group,and their objective voice indexes were collected and compared with those in the experimental group.Results There was no relapse of carcinoma foci in the experimental group within1 year after operation and the vocal cords moved well,with incomplete glottal closure and no obvious complications occurred.3 months after operation,the subjective evaluation indexes of voice showed that the voice was still moderately abnormal,and the objective indexes showed that the voice was worse than before operation.However,6 months after operation,the basic features of the vocal cords gradually recovered.And 1 year after operation,the degree of hoarseness was improved than that before operation,but the voice could not return to a normal voice state.Conclusion In the treatment of early glottic carcinoma,the low-temperature plasma radiofrequency ablation under suspension laryngoscope can help to achieve a better recovery of voice function,which is worthy of wide application in clinic.
