膝关节镜下有限清理与广泛清理术治疗膝关节骨关节炎的效果比较

摘　　要：目的比较关节镜下有限清理术与关节镜下广泛清理术治疗膝关节骨性关节炎(KOA)的效果。方法选取天津市第四中心医院2019年3月至2021年3月收治的130例KOA患者作为研究对象,按照随机数字表法分为研究组与对照组,每组65例。研究组KOA患者行关节镜下有限清理术治疗,对照组KOA患者行关节镜下广泛清理术治疗。记录两组手术情况、手术效果、术后并发症发生情况等指标。结果两组均顺利完成相应手术治疗,研究组手术时间、术后住院时间少于对照组,差异有统计学意义(P<0.05)。两组术前Lysholms评分比较差异未见统计学意义(P>0.05);术后两组Lysholms评分均较之前显著提高(P<0.05),研究组术后Lysholms评分高于对照组(P<0.05)。研究组术后并发症发生率(3.08%,2/65)低于对照组(16.92%,11/65),差异有统计学意义(χ^(2)=5.470,P<0.05)。结论应用关节镜下有限清理术治疗KOA效果显著、安全性高。Objective To compare the effects of limited debridement under arthroscopy and extensive debridement under arthroscopy on knee osteoarthritis(KOA).Methods A total of 130 patients with koa treated in Tianjin Fourth Central Hospital from March 2019 to March 2021 were selected as the research objects.They were randomly divided into study group and control group,with 65 cases in each group.The patients in the study group were treated with limited arthroscopic debridement,and the patients in the control group were treated with extensive arthroscopic debridement.The operation situation,operation effect and postoperative complications of the two groups were recorded.Results Both groups successfully completed the corresponding surgical treatment.The operation time and postoperative hospital stay in the study group were less than those in the control group(P<0.05).There was no significant difference in preoperative lysholms scores between the two groups(P>0.05).Lysholms scores in both groups were significantly higher than before(P<0.05).The postoperative lysholms score of the study group was higher than that of the control group(P<0.05).The incidence of postoperative complications in the study group(3.08%,2/65)was lower than that in the control group(16.92%,11/65),the difference was significant(χ^(2)=5.470,P<0.05).Conclusions Limited debridement under arthroscopy is effective and safe in the treatment of KOA.
