摘　　要：目的:探究富含血小板血浆凝胶技术联合负压封闭引流对上肢大面积皮肤脱套伤患者疼痛程度、瘢痕增生的影响。方法:选取87例上肢大面积皮肤脱套伤患者作为研究对象,分成对照组和观察组。对照组采用VSD治疗,观察组采用VSD联合PRP凝胶技术治疗。对比两组患者皮肤创面愈合情况、疼痛程度、瘢痕增生程度,以及术后不良反应发生情况。结果:观察组皮肤创面愈合情况优于对照组(P<0.05);观察组术后1、2、4周VAS评分均低于对照组(P<0.05);观察组创面完全愈合后1、2个月VSS评分均低于对照组,两组比较差异有统计学意义(P<0.05);两组术后不良反应发生率比较,差异无统计学意义(P>0.05)。结论:针对上肢大面积皮肤脱套伤采用PRP凝胶技术联合VSD治疗,可有效改善患者皮肤愈合情况、瘢痕增生程度,减轻患者疼痛程度。Objective:To investigate the effects of platelet-rich plasma(PRP)gel technique combined with vacuum sealing drainage(VSD)on the pain degree and scar hyperplasia in patients with large-area upper extremity skin degloving injury.Methods:87 patients with large area skin degloving injury of upper limbs were selected as research objects,who were divided into the control group and observation group.The control group was treated with VSD,while the observation group as treated with VSD combined with PRP gel technique.Skin wound healing,pain degree,scar hyperplasia degree and postoperative adverse reactions were compared between patients in the two groups.Results:Wound healing of the observation group was better than that of the control group(P<0.05);VAS score of observation group was lower than that of control group at the 1;week,the 2;week and the 4;week and 4 week after operation(P<0.05);VSS score of the observation group was lower than that of the control group at the 1;month and 2;month after wound healing,and the difference was statistically significant(P<0.05);and there was no significant difference in the incidence of postoperative adverse reactions between the two groups(P>0.05).Conclusion:PRP gel technique combined with VSD can effectively improve the skin healing,scar hyperplasia and pain degree of patients with large area skin degloving injury of upper limbs.
