摘 要:目的探讨生物瓣膜置换术联合托拉塞米治疗心脏瓣膜病的效果及对心肌损伤标志物、CTGF、Lp-PLA2表达的影响。方法择取2019年8月至2020年11月我院收治的84例心脏瓣膜病患者为研究对象,按照治疗方案将其分为对照组和观察组,各42例。对照组采取生物瓣膜置换术治疗,观察组采取生物瓣膜置换术联合托拉塞米治疗。比较两组的临床疗效、心肌损伤标志物、Lp-PLA2、CTGF水平及血管内皮功能指标。结果观察组的治疗总有效率为95.24%,高于对照组的80.95%(P<0.05)。治疗后,两组的CK-MB、cTnI、MYO水平均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组的Lp-PLA2、CTGF水平均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组的ET-1、vWF水平均较治疗前降低,NO水平均较治疗前升高,且观察组优于对照组(P<0.05)。结论生物瓣膜置换术联合托拉塞米治疗心脏瓣膜病的效果显著,可明显降低心肌损伤标志物及CTGF、Lp-PLA2水平,改善血管内皮功能,值得临床推广应用。Objective To explore the effect of biological valve replacement combined with torasemide in the treatment of valvular heart disease and its influences on the expression of myocardial injury markers,CTGF and Lp-PLA2.Methods Eighty-four patients with valvular heart disease treated in our hospital from August 2019 to November 2020 were selected as the research objects.The patients were divided into control group and observation group according to the treatment plan,with 42 cases in each group.The control group was treated with biological valve replacement,and the observation group was treated with biological valve replacement combined with torasemide.The clinical efficacy,myocardial injury markers,Lp-PLA2,CTGF levels and vascular endothelial function indexes were compared between the two groups.Results The total effective rate of treatment in the observation group was 95.24%,which was higher than 80.95%in the control group(P<0.05).After treatment,the levels of CK-MB,cTnI and MYO in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).After treatment,the levels of Lp-PLA2 and CTGF in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).After treatment,the levels of ET-1 and vWF in the two groups were lower than those before treatment,the level of NO was higher than that before treatment,and those in the observation group were better than the control group(P<0.05).Conclusion Biological valve replacement combined with torasemide is effective in the treatment of valvular heart disease.It can significantly reduce the levels of myocardial injury markers,CTGF and Lp-PLA2,and improve the vascular endothelial function,which is worthy of clinical promtion and application.
