摘 要:目的分析慢性阻塞性肺疾病(COPD)患者尿肝型脂肪酸结合蛋白(L-FABP)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、肾损伤分子-1(KIM-1)和β_(2)-微球蛋白(β_(2)-MG)肾损伤标志物的表达水平及临床意义。方法选择2020年1月至2021年6月于九江市第三人民医院治疗的90例COPD患者作为观察组,根据动脉氧分压(PaO_(2))将其分为观察1组(60 mmHg<PaO_(2)≤85 mmHg)、观察2组(40 mmHg<PaO_(2)≤60 mmHg)和观察3组(PaO_(2)≤40 mmHg);同时选取同期九江市第三人民医院的50例体检健康者作为对照组。四组均在晚8时后禁水,留取次日清晨清洁中段尿液标本,L-FABP、NGAL和KIM-1采用酶联免疫吸附法检测,β_(2)-MG采用免疫比浊法检测。比较观察组、各亚组与对照组的尿L-FABP、NGAL、KIM-1和β_(2)-MG水平,分析L-FABP、NGAL、KIM-1和β_(2)-MG与PaO_(2)的相关性。结果观察3组患者的L-FABP、NGAL、KIM-1和β_(2)-MG水平高于观察2组、观察1组、对照组,观察2组患者的L-FABP、NGAL、KIM-1和β_(2)-MG水平高于观察1组、对照组,观察1组患者的L-FABP、NGAL、KIM-1和β_(2)-MG水平高于对照组,差异有统计学意义(P<0.05);Pearson相关性检验结果显示,L-FABP、NGAL、KIM-1和β_(2)-MG与PaO_(2)呈负相关(P<0.05)。结论COPD患者的PaO_(2)与L-FABP、NGAL、KIM-1、β_(2)-MG具有相关性,通过测定尿L-FABP、NGAL、KIM-1和β_(2)-MG水平能够反映肾早期损伤,随着缺氧程度的加重,患者肾功能损伤亦加重,可为COPD的早期干预及治疗提供参考。Objective To analyze the expression levels of kidney injury markers including urinary hepatic fatty acid-binding protein(L-FABP),neutrophil gelatinase-associated lipocalin(NGAL),kidney injury molecule-1(KIM-1)andβ_(2)-microglobulin(β_(2)-MG)in patients with chronic obstructive pulmonary disease(COPD)and their clinical significance.Methods A total of 90 COPD patients who were treated into the Third People′s Hospital of Jiujiang City from January 2020 to June 2021 were selected as the observation group.According to different arterial oxygen partial pressure(PaO_(2)),they were divided into the observation group 1(60 mmHg<PaO_(2)≤85 mmHg),the observation group 2(40 mmHg<PaO_(2)≤60 mmHg),and the observation group 3(PaO_(2)≤40 mmHg).Meanwhile,50 healthy volunteers who underwent physical examinations in the Third People′s Hospital of Jiujiang City during the same period were selected as the control group.All four groups were deprived of water after 8:00 pm,and urine samples were collected from the clean mid-section in the next morning.L-FABP,NGAL,and KIM-1 were detected by enzyme-linked immunosorbent assay,andβ_(2)-MG was detected by immunoturbidimetric method.The levels of urinary L-FABP,NGAL,KIM-1,andβ_(2)-MG in the observation group,each subgroup,and the control group were compared,and the correlation between L-FABP,NGAL,KIM-1,andβ_(2)-MG and PaO_(2) were analyzed.Results The levels of L-FABP,NGAL,KIM-1,andβ_(2)-MG in the observation group 3 were higher than those in the observation group 1,the observation group 2,and the control group,the levels of L-FABP,NGAL,KIM-1,andβ_(2)-MG in the observation group 2 were higher than those in the observation group 1 and the control group,the levels of L-FABP,NGAL,KIM-1,andβ_(2)-MG in the observation group 1 were higher than those in the control group,with statistical differences(P<0.05).Pearson correlation test showed that the levels of L-FABP,NGAL,KIM-1,andβ_(2)-MG were negatively correlated with PaO_(2)(P<0.05).Conclusion PaO_(2) in COPD patients is correlat
