摘  要:目的探讨平面回波图像(echo planar imaging,EPI)与自建模板及微分同胚图像融合(DARTEL)空间配准模板对脑小血管病(cerebral small vessel disease,CSVD)的静息态功能磁共振成像(fMRI)数据分析的影响。方法选择2019年11月~2021年6月首都医科大学宣武医院神经内科门诊确诊的老年CSVD患者36例为观察组,选择同期我院体检的性别、年龄匹配的健康志愿者35例为对照组,采集2组静息态fMRI数据,用EPI和DARTEL 2种空间配准模板分别对数据进行度中心度(DC)分析。分析观察组与对照组比较z值化DC图改变的脑区,采用错误发现率(FDR)校正。结果观察组与对照组在EPI空间配准模板下得到的z值化DC图比较,无统计学差异(t=11.257,FDR校正,P>0.05)。采用DARTEL空间配准模板发现,观察组与对照组比较z值化DC图激活降低的脑区位于双楔叶和左枕上回,DC图激活增强的脑区位于双楔前叶、双后扣带回、双中央旁小叶、左楔叶,结果叠加在蒙特利尔神经研究所标准的T_(1)WI图像上显示,连续体素>10个(FDR校正,P<0.05)。结论对于老年CSVD患者的静息态fMRI数据分析,不同种类的空间配准模板对其结果的影响很大,老年亚裔CSVD患者采用DARTEL模板更适合。Objective To study the effect of echo planar imaging(EPI),self-made template and diffeomorphic anatomical registration through exponentiated lie(DARTEL) spatial normalization template on resting-state fMRI data analysis of cerebral small vessel disease.Methods Thirty-six cerebral small vessel disease patients who visited the Outpatient Department of our hospital from November 2019 to June 2021 served as an observation group and 35 sex-and age-matched volunteers who underwent physical examination in our hospital during the same period served as a control group.Their resting state fMRI data were recorded.The degree centrality(DC) of resting state fMRI data was analyzed by EPI and DARTEL spatial normalization template respectively.The brain regions with changed DC in two groups were analyzed and adjusted by the FDR.ResultsNo significant difference was detected in z-valued DC distribution graph achieved by EPI spatial normalization templete between observation group and control group(t=11.257,FDR-adjusted P>0.05).The resting-state fMRI data analysis of cerebral small vessel disease by DARTEL spatial normalization templete showed that the deceased DC-activated brain regions were located in bilateral cuneus and left superior occipital gyrus while the increased DC-activated brain regions were located in bilateral precuneus, posterior cingulate gyrus, paracentral lobule and left cuneus(consecutive voxel>10,FDR adjusted P<0.05).Conclusion DARTEL spatial normalization template can significantly affect the results of resting-state fMRI data nalysis of cerebral small vessel disease, and is thus more appropriate for the resting-state fMRI data nalysis of Asian cerebral small vessel disease patients.
