摘 要:目的了解2018—2019年北京市门头沟区感染性腹泻细菌病原体的构成及主要致病菌的耐药情况,指导抗生素的正确使用。方法收集2018—2019年北京市门头沟区感染性腹泻病例粪便标本进行沙门菌、志贺菌、致泻性大肠埃希菌(diarrheagenic Escherichia coli,DEC)、副溶血性弧菌、弯曲菌等病原菌的检测,应用微量肉汤稀释法进行沙门菌和DEC的药敏试验,并对病例资料进行统计学分析。结果745份粪便标本检出致病菌218株,其中DEC检出率最高,检出率为12.48%(93/745),其次为沙门菌,检出率为7.11%(53/745)。沙门菌对氨苄西林和萘啶酸的耐药率最高,为66.04%,其次分别为磺胺异噁唑、链霉素和氨苄西林-舒巴坦;DEC对氨苄西林耐药率最高,为65.59%,其次分别为萘啶酸、四环素和磺胺异噁唑。结论北京市门头沟区感染性腹泻病例DEC检出率最高,沙门菌和DEC耐药情况严重,应持续进行监测以掌握耐药谱变化趋势,指导临床正确用药。Objective To understand the composition of bacterial pathogens of infectious diarrhea in Mentougou District and the drug resistance of major causative organisms in Mentougou District,Beijing from 2018 to 2019,so as to guide the correct use of antibiotics.Methods Fecal samples of infectious diarrhea cases in Mentougou District,Beijing from 2018 to 2019 were collected for the detection of Salmonella,Shigella,diarrheagenic Escherichia coli(DEC),Vibrio parahaemolyticus,Campylobacter and other pathogenic bacteria.The drug sensitivity test of Salmonella and DEC was conducted by micro broth dilution method,and the case data were analyzed statistically.Results A total of 218 strains of pathogenic bacteria were detected in 745 fecal specimens.The detection rate of which DEC had the highest with the detection rate of 12.48%(93/745),followed by Salmonella with the detection rate of 7.11%(53/745).The resistance rate of Salmonella to ampicillin and nalidixicillin was 66.04%,followed by sulfamethoxazole,streptomycin and ampicillin-sulbactam,respectively.DEC had the highest resistance rate to ampicillin was 65.59%,followed by nalidixicillin,tetracycline and sulfamethoxazole.Conclusion The detection rate of DEC is the highest among infectious diarrhea cases in Mentougou District,Beijing.The drug resistance of Salmonella and DEC resistance are serious,continuous monitoring should be carried out to master the change trend of drug resistance spectrum and guide the correct drug use in clinic.
