清瘟解毒汤联合改良口腔护理对呼吸机相关性肺炎患者免疫功能及炎症反应的影响

摘　　要：目的分析清瘟解毒汤联合改良口腔护理对呼吸机相关性肺炎患者免疫功能及炎症反应的影响。方法选取2018年3月—2020年5月于重症医学科收治的呼吸机相关性肺炎患者60例,以随机数字表法分为试验组(30例)、对照组(30例)。2组均接受改良口腔护理,对照组接受常规抗感染治疗,试验组在对照组基础上采用清瘟解毒汤治疗,2组均治疗14 d。比较2组疗效、免疫功能、炎症因子。结果试验组总有效率高于对照组(93.33%VS 70.00%,P<0.05)。治疗后与治疗前比较,2组全血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平升高,试验组高于对照组(P<0.05);全血CD8;、血清CRP、PCT、IL-6水平均降低,试验组低于对照组(P<0.05)。结论清瘟解毒汤联合改良口腔护理可改善呼吸机相关性肺炎患者免疫功能,抑制机体炎症反应,疗效确切。Objective To analyze the effect of Qingwen Jiedu decoction combined with improved oral care on immune function and inflammatory reaction of patients with ventilator-associated pneumonia.Methods 60 patients with ventilator-associated pneumonia in our hospital from March 2019 to may 2020 were selected and randomly divided into the experimental group(n=30)and the control group(n=30).Both groups received improved oral care,the control group received conventional anti infection treatment,and the experimental group was treated with Qingwen Jiedu decoction on the basis of the control group.Both groups were treated for 14 d.The efficacy,immune function and inflammatory factors of the two groups were compared.Results The total effective rate of the experimental group was higher than that of the control group(93.33%VS 70.00%,P<0.05).After treatment,the whole blood CD3^(+),CD^(+),CD4^(+)/CD8^(+)levels of the experimental were higher than those of the control group(P<0.05).The whole blood CD8^(+)levels,serum CRP,PCT,IL-6 levels of the experimental group were lower than those of the control group(P<0.05).Conclusion Qingwen Jiedu decoction combined with improved oral care can improve the immune function of patients with ventilator-associated pneumonia,inhibit the body’s inflammatory reaction,and has definite curative effect.
