推陈致新法治疗胆总管复发性结石临床研究

摘　　要：目的探讨推陈致新法治疗胆总管复发性结石的疗效。方法自2018年3月—2020年12月寿光市中医医院门诊就诊的患者中选择符合纳入标准、不符合排除标准的100例胆总管复发性结石的患者,按照简单随机方法分为治疗组(50例)和对照组(50例)。治疗组采用推陈致新法中药治疗,对照组采用金钱胆通颗粒治疗。比较2组治疗前、治疗后第5天、第7天、第2周血清白介素-6(IL-6)、血清白介素-8(IL-8)、胃动素(MOT)、胃泌素(GAS)水平变化、胆总管核磁共振胆胰管成像(MRCP)检测胆总管结石的排出情况。结果2组治疗前血清IL-6、IL-8、MOT、GAS水平比较差别无统计学意义(P>0.05);2组治疗后第5天、第7天、第2周上述指标均较治疗前改善,差异具有统计学意义(P<0.05)。治疗组治疗后第5天、第7天、第2周血清MOT、GAS、IL-6、IL-8水平均较对照组同期有更显著的改善(P<0.05)。MRCP显示治疗后2周以内,治疗组可以更有效地促进结石排出。结论推陈致新法中药通过调控血清MOT、GAS、IL-6、IL-8水平,能够有效地促进胆总管复发性结石排出,对胆总管复发性结石有明显治疗作用。Objective To investigate the effect of evolving new things from the old method in the treatment of recurrent choledocholithiasis.Methods From March 2018 to December 2020,100 patients with recurrent choledocholithiasis from Shouguang Hospital of Traditional Chinese Medicine were randomly divided into treatment group(50 cases)and control group(50 cases).The evolving new things from the old method was used in the treatment group,while the Qianjin Dantong granules was used in the control group.The changes of serum interleukin-6(IL-6),serum interleukin-8(IL-8),motilin(MOT),gastrin(GAS)and choledocholithiasis excretion were compared between the two groups before and 5,7 and 2 weeks after treatment,and choledocholithiasis excretion was detected by choledocholithiasis magnetic resonance cholepancreatic duct imaging(MRCP).Results Before treatment,there were no significant differences in serum IL-6,IL-8,MOT and GAS levels between the two groups(P>0.05).After 5,7 and 2 weeks of treatment,the above indexes in both groups were improved compared with those before treatment,with statistical significance(P<0.05).The levels of MOT,GAS,IL-6 and IL-8 in the treatment group were significantly improved compared with those in the control group at 5,7 and 2 weeks after treatment(P<0.05).MRCP showed that within 2 weeks after treatment,the treatment group could more effectively promote stone expulsion.Conclusion The evolving new things from the old method can effectively remove the choledocholithiasis,and its prossible mechanism was the regulation of serum IL-6,IL-8,MOT and GAS.
