肺结节中医病名及病机探讨

摘　　要：肺结节是在胸部影像上表现为密度增高的实性或亚实性阴影。随着体检普及,肺结节诊断率逐年升高,而部分肺结节随病程增加,演变为肺部肿瘤。中医学并无肺结节对应的病名记载,而有以"肺积""息贲""窠囊"论之,其病因病机认为多由于肺脾虚弱、正气衰惫,邪气内盛、气痰瘀互结所致,治疗上以扶正、攻邪为主,辅以调节气机升降、脏腑功能。目前西医针对早期肺结节尚无特殊治疗,主要采取影像复查随访,而古今中医学家对肺结节的病因病机有独到见解,其学术思想可用以指导肺结节患者进行早期中药干预,延缓疾病进展,缓解患者焦虑。Pulmonary nodules are solid or subsolid shadows with increased density on chest images.With the popularization of physical examinations,the diagnosis rate of lung nodules has increased year by year,and some lung nodules have increased with the course of the disease and evolved into lung tumors.Traditional Chinese medicine of the motherland does not have a record of the disease name corresponding to“pulmonary nodules”,but“feiji”“xiben”and“kenang”.It is caused by the internal accumulation of evil qi and the mutual accumulation of qi,phlegm and stasis.The treatment is mainly to strengthen the body and fight evil,supplemented by regulating the functions of the Qi machine and the viscera.At present,western medicine has no special treatment for early pulmonary nodules,and mainly adopts imaging review and follow-up.Ancient and modern traditional Chinese medicine practitioners have unique insights into the etiology and pathogenesis of pulmonary nodules.Their academic ideas can be used to guide patients with lung nodules in early intervention with traditional Chinese medicine to delay the disease progresses and relieves patient anxiety.
