摘　　要：目的探讨自拟益肾活血方在难治性宫腔粘连(IUA)宫腔镜下宫腔粘连分离术(TCRA)术后患者中的临床疗效。方法选择2019年6月至2021年3月安徽省铜陵市中医医院收治的难治性IUA患者118例,依照随机数字表法将其分为对照组和观察组,各59例。两组均行TCRA,对照组术后给予戊酸雌二醇片联合黄体酮胶囊治疗,在对照组的基础上,观察组给予益肾活血方治疗,均治疗3个月。治疗后进行临床疗效评价,比较两组治疗前后IUA评分、中医证候评分、宫腔容积、子宫内膜厚度、阻力指数(RI)、搏动指数(PI)及血清转化生长因子-β1(TGF-β1)、基质金属蛋白酶抑制因子1(TIMP-1)水平。比较两组治疗期间不良反应的发生情况。结果观察组临床疗效优于对照组,差异有统计学意义(P<0.05)。两组治疗后IUA评分、中医证候评分、RI、PI以及血清TGF-β1、TIMP-1水平均低于治疗前,且观察组低于对照组,而两组治疗后宫腔容积、子宫内膜厚度高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论自拟益肾活血方辅助用于难治性IUA TCRA术后患者的治疗,能够提高疗效,减轻症状,增加子宫内膜厚度,调节体内纤维化因子表达。Objective To investigate the clinical efficacy of self-formulated Yishen Huoxue Prescription for patients with intra uterine adhesions(IUA)after trans cervical resection adhesions(TCRA)operation.Methods A total of 118 patients with refractory IUA admitted to Tongling Hospital of Traditional Chinese Medicine in Anhui Province from June 2019 to March 2021 were selected,and they were divided into control group and observation group according to random number table method,with 59 cases in each group.Both groups were treated with TCRA.The control group was treated with estradiol Valerate Tablets combined with Progesterone Capsules after operation.On the basis of the control group,the observation group was treated with Yishen Huoxue Prescription for three months.The clinical efficacy was evaluated after treatment.IUA score,traditional Chinese medicine syndrome score,uterine cavity volume,endometrial thickness,resistance index(RI),pulbeat index(PI)and serum transforming growth fanctor-β(TGF-β1)and tissue inhibitor of metalloproteinase-1(TIMP-1)levels were compared between the two groups before and after treatment.The incidence of adverse reactions during treatment was compared between the two groups.Results The clinical efficacy of observation group was better than control group,the difference was statistically significant(P<0.05).After treatment,IUA score,traditional Chinese medicine syndrome score,RI,PI,serum TGF-β1 and TIMP-1 levels in two groups were lower than before,and which of the observation group were lower than those of control group,while uterine cavity volume and endometrial thickness in two groups were higher than before,and observation group was higher than control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Self-formulated Yishen Huoxue prescription for patients with IUA after TCRA operation can improve the curative effect,relieve symptoms,increase endometrial thickness,
