摘　　要：目的通过观察针刀结合星状神经节阻滞术对颈性眩晕(CV)患者的临床疗效,为CV的治疗和研究提供思路和方法。方法选取2020年1月至2021年1月就诊于石家庄市中医院CV患者60例,采用随机数字表法将其分为对照组和治疗组,每组各30例。对照组治疗采用星状神经节阻滞术,每周治疗1次,共治疗2次。治疗组在对照组的基础上加用针刀治疗,每周治疗1次,共治疗2次。两组均于治疗前后行颈性眩晕症状与功能评估量表(ESCV)、经颅多普勒超声(TCD)检测,并对两组治疗前、后的数据进行比较,观察比较两组临床疗效果。结果治疗前两组ESCV评分、平均血流速度、收缩期峰值血流速度、舒张末期血流速度比较,差异无统计学意义(P>0.05),治疗后两组ESCV评分、平均血流速度、收缩期峰值血流速度、舒张末期血流速度检测较治疗前升高,且治疗组均高于对照组,差异均有统计学意义(P<0.05)。治疗组临床疗效优于对照组,差异有统计学意义(P<0.05)。结论针刀结合星状神经节阻滞术治疗CV效果显著,有临床推广意义。其长远期疗效有待临床进一步验证。Objective To observe the clinical effect of acupotomy combined with stellate ganglion block on cervical vertigo(CV)patients,and to provide ideas and methods for the treatment and research of CV.Methods A total of 60 CV patients admitted to Shijiazhuang Hospital of Traditional Chinese Medicine from January 2020 to January 2021 were selected and they were divided into control group and treatment group by random number table method,with 30 cases in each group.The control group was treated with stellate ganglion block once a week,twice in total.The treatment group was treated with acupotomy once a week on the basis of the control group,twice in total.The cervical vertigo symptoms and functional assessment scales(ESCV)and transcranial doppler(TCD)were performed in both groups before and after treatment,and the data of the two groups were compared before and after treatment.The clinical efficacy of the two groups was observed and compared.Results Before treatment,there were no significant differences in ESCV score,mean blood flow velocity,peak systolic blood flow velocity,and end diastolic blood flow velocity between the two groups(P>0.05).After treatment,ESCV score,mean blood flow velocity,peak systolic blood flow velocity,and end diastolic blood flow velocity in the two groups were higher than before treatment,and the treatment group was higher than the control group,the differences were statistically significant(P<0.05).The clinical efficacy of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Acupotomy combined with stellate ganglion block is effective in the treatment of CV and has clinical significance.Its long-term efficacy needs further clinical verification.
