摘  要:肠道微生物群在健康宿主体内参与多种稳态调节,包括免疫功能和肠道屏障等。最近多项研究证实了肠道微生物群和脓毒症之间的重要联系。本文从以下三方面重点叙述肠道微生物参与脓毒症发生发展过程的潜在机制:第一,致病性肠道菌群的激活,启动免疫系统引起炎症反应,及有益微生物产物的产生减少等病理改变,影响了宿主对脓毒症的易感性;第二,脓毒症导致肠道微生物群的破坏,诱发和加重终末器官功能障碍;第三,基于微生物的治疗策略,即选择性肠道去污和粪便微生物移植等,可能降低脓毒症的发生率,改善脓毒症患者的预后。从肠道微生物的角度干预脓毒症的病理生理过程,可能成为预防或治疗脓毒症的新靶点。The gut microbiome is involved in the regulation of a variety of homeostatic mechanisms in the healthy host including immune function and gut barrier,etc.Recent studies have confirmed the important linkages between the gut microbiome and sepsis.This paper focuses on the potential mechanisms of intestinal microbes involved in the occurrence and development of sepsis from the following three aspects:Firstly,the activation of pathogenic intestinal flora,activation of the immune system and inflammatory response,as well as the reduction of beneficial microbial products,and other pathological changes affect the susceptibility of the host to sepsis;the second,sepsis leads to the destruction of intestinal microbiota and induces and aggravates terminal organ dysfunction;the third,microbial-based treatment strategies,such as selective intestinal decontamination and fecal microbiological transplantation,may reduce the incidence of sepsis and improve the prognosis of patients with sepsis.Intervention in the pathophysiological process of sepsis from the perspective of intestinal microorganisms may be a new target for the prevention or treatment of sepsis.
