摘　　要：目的为有效开展社区药学服务提供参考。方法借鉴三级甲等医院临床药学工作经验,结合现代信息技术,以药师为主体,在天河区石牌街华师社区卫生服务中心开展药物咨询与药学门诊、处方点评与临床用药干预、线上用药指导、用药监控与药品不良反应监测、用药宣传与药学科普,使药师参与家庭医生签约服务,并对其进行培训,优化药学服务人员配置。结果每月不合理处方率由2018年的2.00%~13.00%减少为2020年的0~5.00%;成功实现药品不良反应上报,上报质量较高,关联性较强;线上“华师药圈”咨询患者由不足100人增至200余人;药物咨询患者增多,且由以往咨询用法用量为主转变为更关注药物疗效、药物相互作用、药品不良反应等。药学门诊在社区得以延伸,药师的专业服务能力得以提高。结论开展社区药学服务,主动提供连续、全面、长期的全程药学管理,能有效促进社区患者合理和安全用药。Objective consultation and pharmacy clinic,prescription review and intervention of clinical medication,online medication guidance,medication monitoring and adverse drug reaction monitoring,medication propaganda,pharmaceutical science popularization were carried out in Huashi Community Health Service Center of Shipai Street in Tianhe District,which learned from clinical pharmacy work experience of a grade Ⅲ-A hospital combined with modern information technology,pharmacists were main participants and trained,they were involved in family doctor signing services,then the staffing of pharmaceutical care was optimized.ResultsThe rate of irrational prescriptions reduced from 2.00%-13.00% in 2018 to 0-5.00% in 2020 per month.The report of adverse drug reaction,the high quality of adverse reaction reports,the strong correlation between drugs and adverse drug reaction were achieved successfully.The number of patients for consultations in online"Huashi medicine circle"increased from less than 100 to more than 200.The number of patients for drug consultations increased,while they changed from mainly counseling the usage and dosage of drug to pay more attention to drug efficacy,drug interaction and adverse drug reactions.Pharmacy clinics were extended in the community and the ability about pharmaceutical care of pharmacists were improved.ConclusionDeveloping pharmaceutical care in community and proactively providing continuous,comprehensive,and long-term full-course pharmaceutical management can effectively promote the rational and safe drug use for patients.
