张卫红教授温清并用法治疗粉刺性乳痈经验

摘　　要：张卫红教授擅长应用“温清并用法”辨治粉刺性乳痈,认为粉刺性乳痈具有郁热、耗气、伤阳、瘀滞、缠绵的特点,是导致粉刺性乳痈病程较长且易反复,形成瘘管,迁延难愈,临床治愈较为复杂的关键。张教授针对粉刺性乳痈“肝经郁热、血虚寒凝”的病机特点,确立疏肝清热、温阳补血、散寒通滞的治疗原则。急性期治以疏肝清热,活血通滞,佐以健脾化痰;缓解期注重温阳补血,扶正托毒,结合患者临床病情变化,灵活化裁,使肿块得消,病情向愈。附验案1则以佐证。Professor Zhang Weihong is good at applying use“warming and clearing simultaneously”to distinguish and treat comedogenic mammary carbuncle.She believes that comedogenic mammary carbuncle has the characteristics of heat stasis,gas consumption,yang injury,stasis and lingering,which is the key to cause comedo carbuncle to have a long and repeated course,form fistulas,prolong and cure complicated clinically.Aiming at the pathogenesis characteristics of comedogenic mammary carbuncle,“liver meridian stagnation,blood deficiency and cold coagulation”,Professor Zhang established the therapeutic principles of soothing liver and clearing heat,warming yang and enriching blood,dispersed-cold and phlegm-dampness.In acute stage,treatment should relieve the liver heat,blood circulation stagnation,with invigorate the spleen and phlegm.In the remission stage,the patient should pay attention to warm yang,nourish blood,support toxin,and combine with the changes of the patient's clinical condition,so as to eliminate the mass and cure the disease.One case is attached for evidence.
