摘　　要：目的:探讨经阴道超声检查诊断深部浸润型子宫内膜异位症的临床应用价值。方法:选取佛山市妇幼保健院2021年1月—8月期间接诊的42例深部浸润型子宫内膜异位症患者进行回顾性分析,对比患者术前经阴道超声检查(TVUS)及术后病理结果,探讨分析经阴道超声检查在诊断DIE中的特异度、敏感度及准确性。结果:42例经手术和病理结果证实DIE患者中,累及子宫骶韧带37例,敏感度94.74%,特异度75.00%,K值为0.63;29例累及直肠,敏感度81.25%,特异度70.00%,K值为0.46;5例累及直肠阴道隔,灵敏度42.86%,特异度94.29%,K值为0.42;7例累及阴道壁,敏感度80.00%,特异度91.89%,K值为0.61;8例累及膀胱后壁,敏感度87.50%,特异度97.06%,K值为0.85。结论:经阴道超声检查诊断深部浸润型子宫内膜异位症具有较高的准确性,有利于为患者提供有效的临床指导,具有广阔的临床应用前景,值得推广应用。Objective To explore the clinical value of transvaginal ultrasonography in the diagnosis of deep invasive endometriosis(DIE).Methods 42 patients with deep invasive endometriosis treated in our hospital from January 2021 to August 2021 were analyzed retrospectively.The preoperative transvaginal ultrasonography(TVUS) and postoperative pathological results were compared,and the specificity,sensitivity and accuracy of TVUS in the diagnosis of DIE were discussed and analyzed.Results Among 42 patients with DIE confirmed by operation and pathology,37 cases involved uterosacral ligament,with sensitivity of 94.74%,specificity of 75.00%and K value of 0.63;29 cases were with rectal involvement,with sensitivity of 81.25%,the specificity of 70.00%,and the K value of 0.46;5 cases involved the rectovaginal septum,with sensitivity of 42.86%,the specificity of 94.29%,and the K value of 0.42;7 cases involved vaginal wall,with sensitivity of 80.00%,the specificity of 91.89%,and the K value of 0.61;8 cases involved the posterior wall of the bladder,with sensitivity of 87.50%,the specificity of 97.06%,and the K value of 0.85.Conclusion Transvaginal ultrasonography has high accuracy in the diagnosis of deep invasive endometriosis,is conducive to provide effective clinical guidance for patients,has broad clinical application prospects,and is worthy of popularization and application.
