摘  要:目的:探讨针对罹患急性缺血性脑卒中的患者运用磁敏感加权成像(susceptibility weighted imaging,SWI)技术对脑内微出血诊断的价值。方法:选取2019年6月—2021年6月阳谷县人民医院收治的罹患急性缺血性脑卒中且疑似脑内微出血的患者50例,运用磁敏感加权成像技术实施检查,并就检查结果与常规MRI序列扫描结果进行比较,对比不同检查方法对脑内微出血的检出率。同时,以临床综合诊断作为金标准,计算不同诊断方法对脑内微出血的诊断效能,并分析影像特征。结果:50例急性缺血性脑卒中疑似脑内微出血的患者,经临床综合诊断,脑内微出血确诊43例(86.00%);SWI序列检出率86.00%居更高水平,且其与临床综合诊断结果比较差异无统计学意义(P>0.05)。以临床综合诊断结果为金标准,就不同检查方法对脑内微出血的诊断效能比较,T1WI序列、T2WI序列、矢状位T2加权成像序列诊断灵敏度、特异度、准确度分别为(20.93%,85.71%,30.00%)、(27.91%,71.43%,34.00%)、(41.86%,85.71%,48.00%),SWI序列诊断效能均为100.00%。SWI诊断灵敏度、准确度与其他检测方法比较,差异具有统计学意义(P <0.05)。结论:针对临床收治的急性缺血性脑卒中,运用磁敏感加权技术实施诊断,有较高的诊断效能。Objective To explore the value of using susceptibility weighted imaging(SWI) in the diagnosis of intracerebral microbleeds in patients with acute ischemic stroke.Methods 50 patients with acute ischemic stroke and suspected intracerebral microbleeds treated in Yanggu County People’s Hospital from June 2019 to June 2021 were selected,using susceptibility weighted imaging(SWI) technique,conducting inspection and test results comparing with conventional MRI sequence scan results,comparing different examination methods for the detection of intracerebral microbleeds.At the same time,the clinical comprehensive diagnosis was used as the gold standard to calculate the diagnostic efficacy of different diagnostic methods for intracerebral microbleeds and analyze the imaging characteristics.Results In 50 patients with acute ischemic stroke suspected of intracerebral microbleeds,43 cases(86.00%) were diagnosed with intracerebral microbleeds after clinical comprehensive diagnosis.The SWI sequence detection rate of 86.00% was equal to that of clinical comprehensive diagnosis(P > 0.05).Based on the gold standard of clinical comprehensive diagnosis results,the diagnostic efficacy of different examination methods for intracranial microbleeds was compared.The diagnostic sensitivity,specificity and accuracy of T1WI sequence,T2WI sequence and sagittal T2-weighted imaging sequence were(20.93%,85.71%,30.00%),(27.91%,71.43%,34.00%),(41.86%,85.71%,48.00%),respectively,SWI sequence diagnosis efficiency were 100.00%.The sensitivity and accuracy of SWI were significantly different from other detection methods(P < 0.05).Conclusion For acute ischemic stroke in clinical treatment,the use of susceptibility weighted imaging technology to diagnose has high diagnostic efficiency.
