生化汤联合心理干预对产后胎盘残留产妇子宫内膜厚度及炎性反应的影响

摘　　要：目的探究加味生化汤联合心理干预对产后胎盘残留产妇子宫内膜厚度及炎性反应的影响。方法选取2017年9月—2020年8月江西省妇幼保健院收治的产后胎盘残留产妇64例为研究对象,按随机数字表法分为对照组和观察组,各32例。对照组给予益母草颗粒,观察组在对照组的基础上施以加味生化汤,2组均连续治疗30 d。2组产妇在治疗期间均进行心理干预。比较治疗后2组产妇的治疗效果及治疗前后的胎盘残留面积、子宫内膜厚度、血清β-HCG水平、外周血WBC计数、VAS评分和血清CRP水平。结果治疗后观察组总有效率高于对照组;与治疗前比,治疗后2组产妇胎盘残留面积、子宫内膜厚度、血清β-HCG水平、外周血WBC计数、VAS评分、血清CRP水平均降低,且观察组低于对照组(均P<0.05)。结论加味生化汤联合心理干预能明显降低产后胎盘残留产妇的子宫内膜厚度,减轻疼痛程度,同时抑制炎性反应,且效果显著。Objective To explore the effect of modified Shenhua decoction combined with psychological intervention on endometrial thickness and inflammatory response in parturient women with placenta residue.Methods 64 cases of postpartum placenta residue in Maternal and Child Health Hospital from September 2017 to August 2020 were selected as the research objects,and were divided into control group and observation group according to random number table method,with 32 cases in each group.The control group was given Leonurus granules,and the observation group was given modified Shenhua decoction on the basis of the control group.Both groups were treated continuously for 30 days.Psychological intervention was performed in both groups during the treatment period.The therapeutic effect,placental residual area,endometrial thickness,serumβ-HCG level,peripheral blood WBC count,VAS score and serum CRP level before and after treatment were compared between the two groups.Results The total effective rate of observation group was higher than control group after treatment.Compared with before treatment,placental residual area,endometrial thickness,serumβ-HCG level,peripheral blood WBC count,VAS score and serum CRP level were all decreased in the two groups after treatment,and the observation group was lower than the control group(all P<0.05).Conclusion Modified Shenhua decoction combined with psychological intervention can significantly reduce the endometrial thickness of postpartum placenta residue parturients,relieve pain,and inhibit inflammatory response,and has significant effect.
