摘　　要：目的分析参附注射液联合基于ABCDEF集束化护理策略对重症肺炎患者血管内皮功能及凝血状态的影响。方法选取60例2018年4月—2020年8月于南昌大学第四附属医院重症医学科就诊的重症肺炎患者,以随机数字表法分为A组、B组,各30例。A组予以氢化可的松注射液静脉滴注,B组在A组基础上予以参附注射液静脉滴注,2组均治疗2周,且治疗期间均予以基于ABCDEF集束化护理策略干预。观察2组临床结局,比较2组治疗前、治疗2周后的血管内皮功能及凝血状态。结果B组住ICU、拔管、机械通气、住院时间短于A组(P<0.05)。治疗2周后,2组血清s E-selectin、v WF及血浆D-D水平低于治疗前,且B组低于A组(P<0.05);外周血APTT短于治疗前,B组短于A组(P<0.05);外周血PLT水平高于治疗前,B组高于A组(P<0.05)。结论参附注射液联合基于ABCDEF集束化护理策略可改善重症肺炎患者血管内皮功能及凝血状态,进而改善患者临床结局。Objective To analyze the influence of Shenfu injection combined with ABCDEF-based cluster nursing strategy on vascular endothelial function and coagulation status in patients with severe pneumonia.Methods 60 patients with severe pneumonia who were treated in the department of intensive care medicine of the Fourth Affiliated Hospital of Nanchang University from April 2018 to August 2020 were divided into the group A and the group B by random number table method,with 30 cases in each group.The group A was given hydrocortisone injection by intravenous drip,and the group B was given Shenfu injection by intravenous drip on the basis of the group A.Both groups were treated for 2 weeks,and they were intervened by ABCDEF cluster nursing strategy during the treatment.The clinical outcomes of the two groups were observed,and the vascular endothelial function and coagulation status of the two groups were compared before and after treatment for 2 weeks.Results The length of stay in ICU,extubation,mechanical ventilation and hospitalization in the B group was shorter than that in the group A( P < 0.05).After 2 weeks of treatment,the levels of serum s E-selectin,v WF and plasma D-D in the two groups were lower than those before treatment,and those in the group B were lower than those in the group A( P < 0.05).APTT in peripheral blood was shorter than that before treatment,and the B group was shorter than that in the group A( P < 0.05).The level of PLT in peripheral blood was higher than that before treatment,and that in group B was higher than that in group A( P < 0.05).Conclusion Shenfu injection combined with ABCDEF-based cluster nursing strategy could improve the vascular endothelial function and coagulation status of patients with severe pneumonia,and then improve the clinical outcome of patients.
