静脉通路护理门诊质量管理标准在PICC置管患者中的应用研究

摘　　要：目的:将《静脉通路护理门诊质量管理标准》应用于市内二级及以上医疗机构静脉通路护理门诊,评价其应用效果。方法:便利选取2018年10月至12月就诊的280例患者为对照组,选取2019年10月至12月就诊的280例患者为干预组。干预组采用标准化培训方案实施《静脉通路护理门诊质量管理标准》。对照组按照常规方法落实日常管理要求。采用肿瘤患者PICC自我管理能力量表分别调查两组患者的导管自我管理能力,并统计两组患者PICC并发症发生率。结果:干预组PICC导管相关性血流感染发生率1.07%,较对照组的1.79%明显降低,差异有统计学意义(P<0.05)。干预组自我管理能力总分及维护依从性、导管管理信心、异常情况处理3个维度得分均明显高于对照组,差异有统计学意义(P<0.05)。结论:《静脉通路护理门诊质量管理标准》的应用,有利于规范护士行为,对带管患者实施标准化的维护服务,能提高PICC置管患者自我管理能力,降低并发症的发生,从而降低患者医疗成本。Objective:To evaluate the application effect of quality management standards for intravenous access care clinics in Secondary and above medical institutions in Tianjin.Methods:A total of 280 patients who visited intravenous access care clinics from October to December 2018 were selected as the control group and 280 patients who visited intravenous access care clinics from October to December 2019 were selected as the intervention group.The intervention group adopted standardized training program to implement the quality management standards of intravenous access care clinics.The control group implemented daily management requirements according to the routine care.The self-management ability of catheters in the two groups was investigated with the PICC self-management scale for tumor patients,meanwhile,the incidence of PICC complications in the two groups was calculated.Results:The incidence of PICC catheter-related bloodstream infection in the intervention group was 1.07%,significantly lower than the control group(1.79%).The total score of self-management ability of catheters and the scores of three dimensions including maintenance compliance,confidence in catheter management and abnormal treatment in the intervention group were significantly higher than those in the control group(P<0.05).Conclusion:The application of quality management standards for intravenous access care clinics is helpful to standardize the behaviors of nurses to implement standardized maintenance services for patients with catheters which can improve the self-management ability of patients and reduce the occurrence of complications,so as to reduce the medical cost of patients.
