家庭无创通气在稳定期COPD合并Ⅱ型呼吸衰竭患者中的疗效评价

摘　　要：目的探讨家庭无创通气在稳定期COPD合并Ⅱ型呼吸衰竭患者中的应用效果。方法选择2019年1月至2020年12月经治疗后稳定期的重度COPD患者84例随机分为对照组(n=41)及无创通气组(n=43)。对照组给予常规治疗及家庭氧疗,无创通气组在常规治疗基础上给予家庭无创通气治疗,随访1年,比较2组患者的mMRc评分、血气分析、肺功能、及CAT评分、年住院次数、住院时间、住院治疗费用、气管插管率以及死亡率。结果与对照组相比,无创通气组治疗后患者mMRc评分、CAT评分、血气分析均有改善(P<0.05),年住院次数明显降低(P<0.05)。肺功能无明显改善(P>0.05),住院治疗费用、气管插管率及死亡率差异无统计学意义(P>0.05)。结论家庭无创通气对于稳定期COPD合并Ⅱ型呼吸衰竭患者能够改善其症状,提高活动耐量,减少住院次数,改善生活质量,但对肺功能无明显改善作用,对住院治疗费用、气管插管率及死亡率无明显改善。Objective To investigate the application effects of non-invasive ventilation in patients with stable COPD complicated by typeⅡrespiratory failure.Methods The patients with severe COPD who were treated in our hospital from January 2019 to December 2020 were enrolled in the study,who were randomly divided into control group(n=41)and observation group(n=43).The patients in control group were given routine treatment combined with home oxygen therapy,however,the patients in observation group,on the basis of routine therapy,were treated by non-invasive ventilation,with 1-year follow up.The mMRc score,blood gas indexes,lung function,CAT score,annual hospitalization frequency,length of hospital stay,cost of hospital treatment,endotracheal intubation rate and death rate were observed and compared between the two groups.Results Compared with those in control group,the mMRc scores,CAT scores and blood gas indexes in observation group were significantly improved(P<0.05),and the annual hospitalization frequency was significantly decreased(P<0.05).However there were no significant differences in the lung function,hospitalization cost,tracheal intubation rate and death rate between the two groups(P>0.05).Conclusion The home non-invasive ventilation can improve the symptoms of patients with stable COPD complicated by typeⅡrespiratory failure,which can increase the activity tolerance,decrease the hospitalization frequency,and improve the life quality of patients,however,which has no obvious effects on the lung function of patients,hospitalization cost,tracheal intubation rate and death rate.
