摘　　要：目的研究硝苯地平联合硫酸镁治疗妊娠期高血压患者时红细胞(HCT)及白蛋白(ALB)指标的变化。方法选取2018年1月至2020年12月收治的子痫前期患者80例,分为对照组和研究组,每组40例。对照组单纯采用硫酸镁治疗,研究组采用硝苯地平联合硫酸镁治疗。2组均治疗10 d,并随访至分娩。比较2组治疗前后HCT值、ALB值及其两者差值(HCT-ALB);观察2组治疗前后舒张压(DBP)、收缩压(SBP)变化,对比2组妊娠不良结局发生率。结果治疗后研究组HCT低于对照组,ALB高于对照组,HCT-ALB低于对照组,差异有统计学意义(P<0.05);治疗后研究组SBP、DBP均低于对照组,差异有统计学意义(P<0.05);研究组产后出血、胎儿宫内窘迫、胎儿生长受限、早产等不良结局发生率低于对照组,差异有统计学意义(P<0.05)。结论硫酸镁联合硝苯地平能降低HCT-ALB的差值,改善预后;HCT-ALB可以应用于子痫前期疗效的评估,对指导临床用药有指导意义。Objective To investigate the therapeutic effects of nifedipine combined with magnesium sulfate on preeclampsia,and to explore the changes of red blood cell(HCT)and albumin(ALB)indexes.Methods Eighty patients with preeclampsia who were treated from January 2018 to December 2020 were enrolled in the study,who were randomly divided into control group and research group,with 40 cases in each group.The patients in control group were treated by magnesium sulfate only,however,the patients in research group were treated by nifedipine combined with magnesium sulfate,with a treatment course of 10 days for both groups.The changes of diastolic blood pressure(DBP)and systolic blood pressure(SBP)before and after treatment,and the incidence of adverse pregnancy outcomes were observed and compared between the two groups.Results After treatment,the levels of HCT,SBP,DBP in research group were significantly lower than those in control group,however,the ALB levels were significantly higher than those in control group(P<0.05).Moreover the incidence rates of postpartum hemorrhage,fetal distress,fetal growth restriction,preterm delivery and other adverse outcomes in research group were significantly lower than those in control group(P<0.05).Conclusion Magnesium sulfate combined with nifedipine can decrease the difference of HCT-ALB,so as to improve the prognosis of patients with preeclampsia.HCT-ALB can be used in the evaluation of therapeutic effects on preeclampsia,which has guiding significance for clinical medication.
