全程陪伴舒适体位联合间苯三酚在初产妇经阴分娩中的临床应用效果

摘　　要：目的探讨全程陪伴舒适体位联合间苯三酚在初产妇经阴分娩中的临床应用效果。方法选取在菏泽市妇幼保健计划生育服务中心及菏泽市立医院自然分娩的初产妇160例,随机分为对照组和观察组,每组80例。观察组产妇在自然临产后宫口开大3 cm时给予80 mg间苯三酚静脉推注配合全程陪伴的舒适体位分娩。对照组采用传统平卧位或侧卧位待产,传统膀胱截石位分娩。比较两组初产妇的产程时间、分娩结局及并发症情况。结果观察组第一产程、第二产程和总产程显著短于对照组(P<0.05);阴道分娩成功率高于对照组(P<0.05);宫颈裂伤、产后2 h出血量均少于对照组(P均<0.05)。两组产妇的第三产程、新生儿Apgar评分比较,P均>0.05。结论医护人员应鼓励初产妇家属全程陪伴舒适体位分娩,联合使用间苯三酚更有利于促进产程进展,提高阴道分娩率,减少分娩并发症的发生。Objective To explore the clinical effect of complete companion and comfortable position combined with phloroglucinol on primipara vaginal delivery.Methods 160 primiparas who gave birth naturally in Heze Maternal-child Health and Family Planning Service Center and Heze Municipal Hospital were selected and randonly divided into the observation group and the control group,80 cases in each group.The observation group were given 80 mg phloroglucinol intravenous injection and accompanied by comfortable posture.The control group lied in traditional supine position or lateral position,and gave birth in traditional bladder lithotomy position.The duration,outcome of delivery and complications were compared between the two groups.Results The first stage of labor,the second stage of labor and the total stage of labor in the observation group were significantly shorter than those in the control group(P<0.05).The success rate of vaginal delivery in the observation group was higher than that in the control group,while the cervical laceration rate and postpartum hemorrhage after 2 h were lower than those in the control group(P<0.05).There was no significant difference in the third stage of labor and Apgar score between the two groups(P>0.05).Conclusion Healthcare workers should encourage parturients to give birth in a comfortable position accompanied by their family member.Combined use of phloroglucinol is more conducive to promote the progress of labor,increase the rate of vaginal delivery and reduce the occurrence of complications during delivery.
