摘 要:目的 观察支气管哮喘患者外周血单个核细胞中干扰素调节因子-1(interferon regulatory factor-1, IRF1)表达情况,探讨其与患者对糖皮质激素治疗敏感性的关系。方法 哮喘患者118例均给予醋酸泼尼松治疗,根据患者对糖皮质激素的敏感性分为敏感组76例和不敏感组42例,另选取同期体检健康者60例为对照组。采用实时荧光定量PCR法检测治疗前3组外周血单个核细胞IRF1 mRNA相对表达量,采用Western blot法检测治疗前3组外周血单个核细胞IRF1蛋白相对表达量,采用ELISA法检测支气管哮喘患者治疗前血清肿瘤坏死因子-α(tumor necrosis factor-α, TNF-α)和白细胞介素-6(interleukin-6, IL-6)水平,比较敏感组与不敏感组体质量指数、血清TNF-α、IL-6水平等临床资料,比较3组外周血单个核细胞IRF1 mRNA和蛋白相对表达量,采用多因素logistic回归分析支气管哮喘患者糖皮质激素治疗不敏感的影响因素。结果 不敏感组治疗前血清IL-6[(23.39±5.98)ng/L]、TNF-α[(15.26±4.06)ng/L]水平及过敏性鼻炎比率(42.86%)、体质量指数<18 kg/m^(2)比率(14.29%)、体质量指数>24 kg/m^(2)比率(45.24%)均高于敏感组[(17.77±4.31)ng/L、(11.75±2.47)ng/L、23.68%、11.84%、18.42%](P<0.05),体质量指数18~24 kg/m^(2)比率(40.48%)低于敏感组(69.74%)(P<0.05);不敏感组治疗前外周血单个核细胞IRF1 mRNA(1.89±0.31)和蛋白(2.01±0.35)相对表达量均高于敏感组(1.56±0.29、1.84±0.27)、对照组(1.20±0.23、0.97±0.18)(P<0.05),敏感组均高于对照组(P<0.05);IL-6(OR=1.356,95%CI:1.143~1.608,P<0.001)、TNF-α(OR=1.263,95%CI:1.060~1.505,P=0.009)、外周血单个核细胞IRF1蛋白相对表达量(OR=11.489,95%CI:2.597~50.821,P=0.001)、体质量指数>24 kg/m^(2)(OR=3.500,95%CI:1.066~11.496,P=0.039)是支气管哮喘患者糖皮质激素治疗不敏感的影响因素。结论 支气管哮喘患者外周血单个核细胞IRF1 mRNA和蛋白表达增高,外周血单个核细胞IRF1、血清TNF-α和IL-6Objective To explore the expression of interferon regulatory factor-1(IRF1) in peripheral mononuclear cells, and to investigate its correlation with the sensitivity of bronchial asthma to glucocorticoid(GC). Methods Totally 118 bronchial asthma patients were treated with prednisone and were divided into sensitivity group(n=76) and insensitivity group(n=42) according to the sensitivity to GC. Another 60 healthy volunteers were as controls(control group). Before treatment, the relative expression of IRF1 mRNA in peripheral mononuclear cells was detected by real-time fluorescence quantitative PCR, the relative expression of IRF1 protein in peripheral mononuclear cells was detected by Western blot in three groups, and the serum tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) levels were detected by ELISA in patients with bronchial asthma. The general data as body mass index(BMI), and the levels of serum TNF-α and serum IL-6 were compared between sensitivity group and insensitivity group, and the relative expressions of IRF1 mRNA and protein were compared in three groups. Multivariate logistic regression was performed to analyze the influencing factors of insensitivity to GC in patients with bronchial asthma. Results The levels of IL-6 and TNF-α before treatment, as well as the percentages of patients with of allergic rhinitis, BMI<18 kg/m^(2)and BMI>24 kg/m^(2)were higher in insensitivity group [(23.39±5.98) ng/L,(15.26±4.06) ng/L, 42.86%, 14.29%, 45.24%] than those in sensitivity group [(17.77±4.31)ng/L,(11.75±2.47)ng/L,23.68%,11.84%,18.42%](P<0.05),and the percentage of patients with BMI of 18to 24kg/m2 was lower in insensitivity group(40.48%)than that in sensitivity group(69.74%)(P<0.05).The relative expressions of IRF1 mRNA and protein before treatmen were higher in insensitivity group(1.89±0.31,2.01±0.35)than those in sensitivity group(1.56±0.29,1.84±0.27)and control group(1.20±0.23,0.97±0.18)(P<0.05),and higher in sensitivity group than those in control group(P<0.05).IL-6(OR=1.356,95%CI:1.1
