摘 要:目的探讨磁共振成像(MRI)扫描联合三维动脉自旋标记成像(3D-ASL)对胶质瘤术后复发的诊断价值。方法选取本院2016年7月至2018年7月收治的82例行手术治疗的胶质瘤患者作为研究对象,所有患者均行MRI扫描和3D-ASL检查,比较单纯MRI扫描和MRI联合3D-ASL的疾病诊断结果。结果MRI扫描联合3D-ASL对术后复发的检出率为94.23%,放射性脑损伤检出率为93.33%,均高于MRI扫描的80.77%和73.33%,差异有统计学意义(P<0.05);MRI扫描联合3D-ASL对胶质瘤术后复发的诊断敏感性为94.23%,特异性为93.33%,准确性为93.90,漏诊率为5.77%,误诊率为6.67%,MRI扫描对胶质瘤术后复发的诊断敏感性、特异性、准确性、漏诊率和误诊率分为80.77%、73.33%、78.05%、19.23%和26.67%,组间比较差异有统计学意义(P<0.05);术后复发患者对侧镜像区、对侧白质和对侧灰质最大相对脑流量明显高于放射性脑损伤患者,组间比较差异有统计学意义(P<0.05)。结论MRI联合3D-ASL可准确诊断胶质瘤术后复发,在术后复发和放射性脑损伤的鉴别诊断中发挥重要作用,值得临床推广应用。Objective To explore the diagnostic value of magnetic resonance imaging(MRI)combined with three dimensiona-arterial spin label-ing(3D-ASL)for postoperative recurrence of glioma.Methods 82 patients with glioma treated in our hospital from July 2016 to July 2018 were se-lected as the research subjects.All patients underwent MRI scanning and 3D-ASL examination.The results of MRI scanning alone and MRI com-bined with 3D-ASL were compared.Results The detection rate of postoperative recurrence by MRI scanning combined with 3D-ASL was 94.23%,and the detection rate of radiation brain injury was 93.33%,which were higher than those by MRI scanning(80.77%and 73.33%,respectively),and the difference was statistically significant(P<0.05).The diagnostic sensitivity,specificity,accuracy,missed diagnosis rate and misdiagnosis rate of MRI scanning combined with 3D-ASL for postoperative recurrence of glioma were 94.23%,93.33%,93.90%,5.77%and 6.67%,respectively.The di-agnostic sensitivity,specificity,accuracy,missed diagnosis rate and misdiagnosis rate of MRI scanning for postoperative recurrence of glioma were 80.77%,73.33%,78.05%,19.23%and 26.67%,respectively,and the difference between groups was statistically significant(P<0.05).The maxi-mum relative brain flow of contralateral mirror area,contralateral white matter and contralateral gray matter in patients with postoperative recurrence was significantly higher than that in patients with radiation brain injury,and the difference between groups was statistically significant(P<0.05).Conclusion MRI scanning combined with 3D-ASL can accurately diagnose postoperative recurrence of glioma,play important role in the differen-tial diagnosis of postoperative recurrence and radiation brain injury,and it is worthy of clinical application.
