摘  要:目的研究维生素D2注射液联合抗生素对社区获得性肺炎(CAP)患儿炎性因子和免疫功能的影响。方法选取2019年10月至2020年10月收治的98例CAP患儿作为研究对象,随机分为对照组和联合组,每组49例。2组均给予抗炎、退热、化痰、止咳等常规治疗。联合组在对照组基础上补充维生素D2。比较2组炎性因子[白介素6(IL-6)、C-反应蛋白(CRP)、降钙素原(PCT)]、T淋巴细胞亚群(CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+))、NK、Th1/Th2及相关细胞因子、免疫球蛋白(IgG、IgA、IgM)、红细胞免疫指标(RBC-C3bR、RBC-ICR)变化。结果2组治疗前IL-6、CRP、PCT、CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+)、NK、Th1、Th2、Th1/Th2、IFN-γ、IL-4、IFN-γ/IL-4、IgG、IgA、IgM、RBC-C3bR、RBC-ICR比较差异无统计学意义(P>0.05);2组治疗后IL-6、CRP、PCT、CD_(8)^(+)、Th2、IL-4、RBC-ICR均显著低于治疗前,CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、NK、Th1、Th1/Th2、IFN-γ、IFN-γ/IL-4、IgG、IgA、IgM、RBC-C3bR均显著高于治疗前,差异有统计学意义(P<0.05),且联合组变化更明显,差异有统计学意义(P<0.05)。结论维生素D2注射液联合抗生素治疗CAP具有良好临床疗效,能够显著减轻炎性反应,增强细胞免疫、体液免疫和红细胞免疫功能。Objective To investigate the effects of vitamin D2 combined with antibiotics on the inflammatory cytokines and immune function in children with community-acquired pneumonia(CAP).Methods A total of 98 children with CAP who were treated in our hospital from October 2019 to October 2020 were enrolled in the study,who were randomly divided into control group(n=49)combination treatment group(n=49).The patients in both groups were treated by basic treatment including antiinfection,antipyretic,expectorant and relieving airway spasm,moreover,the patients in combination group were given vitamin D2.The levels of IL-6,CRP,PCT,CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),NK,Th1/Th2 and related cytokines,IgG,IgA,IgM,RBC-C3bR and RBC-ICR before and after treatment were observed and compared between the two groups.Results Before treatment,there were no significant differences in the levels of IL-6,CRP,PCT,CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(4)^(+)/CD_(8)^(+),NK,Th1,Th2,Th1/Th2,IFN-γ,IL-4,IFN-γ/IL-4,IgG,IgA,IgM,RBC-C3bR and RBC-ICR between the two groups(P>0.05).After treatment,the levels of IL-6,CRP,PCT,CD_(8)^(+),Th2,IL-4 and RBC-ICR in both groups were significantly decreased,however,the levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),NK,Th1,Th1/Th2,IFN-γ,IFN-γ/IL-4,IgG,IgA,IgM and RBC-C3bR were significantly increased(P<0.05),moreover,the decrease degree and increase degree in combination treatment group were more significant than those in control group(P<0.05).Conclusion The vitamin D2 combined with antibiotics in treatment of CAP has better curative effects,which can relieve inflammatory reaction,and improve the cellular immunity,humoral immunity and erythrocyte immune function of patients.
