摘　　要：高血压是老年人的常见疾病,随着病程进展还常伴有心脏和肾脏等器官的损害。高血压对肾脏的损害常由于临床表现不明显被忽视,从而耽误治疗。肾脏是高血压相关损害的重要靶器官,老年高血压具有其特有的特征,多年原发性高血压不断进展常继发肾脏结构和功能损害,甚至可能发展为肾衰竭。众所周知,高血压肾损害的突出特征之一就是尿蛋白异常,此外其他化验指标、影像学检查以及肾脏活检也可提示肾损害的发生。随着医学理念和医疗水平的不断进步,诸多学者也在探索与高血压肾损害相关的灵敏度更高、特异性更强的血清学指标。对于高血压肾损害的治疗,除了良好生活习惯、合理控制血压外,手术及一些新型治疗方案也多次被提出。因此本文将总结并阐述老年高血压特点,以及老年高血压肾损害早期识别、早期干预及治疗的最新进展以提高相关预后。Hypertension is a common disease in the elderly and is often accompanied by damage to the heart and kidneys along with the progression of the disease.The renal damage caused by hypertension is often overlooked due to the lack of obvious clinical manifestations,thus delaying treatment.Kidney is an important target organ for hypertension-related damage.Hypertension in the elderly has its own characteristics,and progressive primary hypertension over many years often leads to renal structural and functional damage,and may even develop into renal failure.As is known to all,one of the prominent features of renal damage in hypertension is abnormal urinary protein.In addition,other laboratory indicators,imaging examinations and kidney biopsy can also indicate the occurrence of renal damage.With the continuous progress of medical concept and medical level,many scholars are also exploring higher sensitivity and specificity of serological indicators related to renal damage in hypertension.For the treatment of hypertensive kidney damage,in addition to good living habits,reasonable control of blood pressure,surgery and some new treatment schemes have been put forward for many times.Therefore,this article will summarize and elaborate the characteristics of senile hypertension,as well as the latest progress in early recognition,early intervention and treatment of senile hypertension renal damage to improve the related prognosis.
