MIS-TLIF术中局部使用地塞米松浸渍速即纱的短期疗效观察

摘　　要：目的探讨MIS-TLIF术中局部使用地塞米松浸渍速即纱的短期临床效果。方法纳入2018年7月~2019年10月本科收治的MIS-TLIF手术患者80例,随机分为A、B两组各40例,A组术中局部使用地塞米松浸渍速即纱,B组使用安慰剂(生理盐水)浸渍速即纱。采用VAS评分评估患者手术前后的腰腿痛程度,采用JOA评分评估功能恢复程度。结果两组患者术后VAS评分及JOA评分均较术前显著改善;A组术后1~3 d腰腿痛VAS评分、术后5 d的JOA评分改善情况均显著优于B组,差异有统计学意义(P<0.05)。两组术后5 d、1个月的VAS评分及术后1个月的JOA评分差异无统计学意义(P>0.05)。两组患者均未发生明显不良反应。结论MIS-TLIF术中局部使用地塞米松浸渍速即纱能有效减轻术后短期疼痛,有利于早期康复锻炼。Objective To explore the short-term clinical effect of dexamethasone combined with Surgicel during MIS-TLIF.Methods From July 2018 to October 2019,80 cases who accepted the treatment of MIS-TLIF admitted in our department were randomly divided into two groups:group A and group B,with 40 cases in each group.Group A was treated with dexamethasone combined with Surgicel during MIS-TLIF,and group B was treated with placebo.The degree of lumbago and leg pain before and after operation was assessed by visual analogue score(VAS),and the degree of functional recovery was assessed by JOA.Results The VAS and JOA score after operation were significantly improved in both two groups compared with before operation(P<0.05).The VAS score at 1-3 d after operation and JOA score at 5 d after operation in group A were better than those in group B(P<0.05).There were no significant differences between the two groups in VAS score at 5 d,1 month after operation and JOA score at postoperative 1 month(P>0.05).No obvious adverse reactions occurred.Conclusion The application of dexamethasone combined with Surgicel during MIS-TLIF can effectively reduce short-term postoperative pain,and is conducive to early rehabilitation exercise.
