摘  要:目的探讨Marchiafava-Bignami disease(MBD)患者的核磁共振扩散张量成像(DTI)的影像学特征。方法选取2016年12月至2020年12月山东省聊城市茌平区第二人民医院收治的30例MBD患者与30例正常志愿者进行DTI扫描,并对各参数进行比较。结果胼胝体膝部、压部、白质纤维束各区域MBD患者平均扩散率(MD)值高于正常组,慢性乙醇中毒髓鞘脱失造成的受限扩散减轻或血管源性水肿造成扩散的增加。慢性乙醇中毒的白质整体完整性的损害,白质纤维束显微结构的断裂,如髓鞘的缺失或者轴索的变性退化,可能让水分子更加自由移动,造成部分各向异性(FA)减少,各向异性降低,各向同性增加,则相对各向异性(RA)减低,FA及RA值均低于正常组。由于胼胝体部纤维多交叉走行,纤维走行一致性较弱,MBD患者、正常组胼胝体体部的MD值、FA值、RA值相差变化不大。MBD患者胼胝体外最易累及区域为皮质下白质,以半卵圆中心最易受累及。结论DTI各参数能够准确评价MBD神经纤维束的病变进展,具有重要的临床应用价值。Objective To explore the image features of diffusion tensor imaging(DTI)in patients with marchiafava-bignami disease(MBD).Methods Thirty MBD patients and 30 normal volunteers were enrolled for DTI scanning in the Second People's Hospital of Chiping District,Liaocheng City,Shandong Province from Dec.2016 to Dec.2020.The parameters were compared.Results The mean diffusivity(MD)values of the genu of corpus callosum,splenium of corpus callosum and white matter fiber bundle were higher in MBD patients than those in normal volunteers.Decreased limited-diffusion caused by demyelination in chronic alcohol intoxicationc or the increased diffusion led by vasogenic edema,the damage of the overall integrity of white matter in chronic alcohol intoxication and the rupture of the microstructure of white matter fiber bundles such as loss of myelin or axonal degeneration may allow water molecules to move more freely,resulting in a decrease in fractional anisotropy(FA),decrease in anisotropy,increase in isotropy,decrease in relative anisotropy(RA)as well as lower FA and RA values than those in the normal group.Because the fibers of the corpus callosum often crossly extend,the consistency of the fiber orientation is weak.MD,FA and RA values of the corpus callosum were not largely changed between MBD patients and normal groups.The subcortical white matter was the most easily affected outside the corpus callosum in MBD patients,and the most easily affected area was the center of the semiovale.Conclusion DTI parameters can accurately evaluate the progression of neurofibrillary tract lesions in MBD and have important clinical application value.
