妊娠合并膀胱子宫内膜异位症1例

摘　　要：子宫内膜异位症是一种常见的妇科病,多见于育龄期女性,可引起慢性盆腔疼痛或者痛经、不孕、结节或包块,对年轻女性生活质量造成严重的影响。因其具有侵蚀性、复发性,被称为“良性癌症”。泌尿系统累及比较罕见(1.0%~5.5%),其主要发生于膀胱(70%~85%)。膀胱子宫内膜异位症的临床症状不典型,起病隐匿,甚至有一部分患者无明显症状,因此容易被误诊,甚至漏诊。2021年滨州医学院附属医院产科收治1例妊娠合并膀胱子宫内膜异位症患者,于剖宫产术中发现,术中误诊为膀胱肿瘤,于剖宫产手术同时行膀胱部分切除术,手术顺利,术后病理诊断为膀胱子宫内膜异位症,现结合国内外文献对该病例进行分析报道,通过该病例报道,为该病以后的诊断及治疗等临床实践提供依据。Endometriosis is a common gynecological disease,which is more common in women of childbearing age.It can cause chronic pelvic pain,dysmenorrhea,infertility,nodules,and masses.It has a serious impact on the quality of life of young women.It is called"benign cancer"because of its aggressiveness and recurrence.Urinary system involvement is relatively rare,accounting for about 1.0%to 5.5%,which mainly occurs in the bladder,accounting for about 70%to 85%.The clinical symptoms of bladder endometriosis are atypical,and onset is hidden,and some patients even have no obvious symptoms,which makes it easy to be misdiagnosed or even missed.Recently,a patient of pregnancy complicated with bladder endometriosis was admitted to the Department of Obstetrics of Binzhou Medical University Hospital.The patient was misdiagnosed with bladder tumor during cesarean section and partial cystectomy was successfully performed at the same time.Postoperative pathological result showed a diagnosis of bladder endometriosis.This case is analyzed and reported in combination with domestic and foreign literature.The case report provides a basis for clinical practice such as the diagnosis and treatment of the disease in the future.
