临床护理路径在髋部骨折围术期预防下肢深静脉血栓形成中的应用

摘　　要：目的分析对髋部骨折使用围术期临床护理路径对预防下肢深静脉血栓形成的效果。方法择取2018年1月至2019年12月杭州市中医院配合本研究的髋部骨折患者52例,根据随机数字表法分为研究组和对照组,每组各26例。研究组采用围术期临床护理路径,对照组采用常规护理,对两组的下肢深静脉血栓形成等并发症,抑郁自评量表(SDS)和焦虑自评量表(SAS)评分、肢体功能评分(FAM)和视觉模拟评分法(VAS)评分、护理满意度进行对比,并评价两组生活质量、骨折复位情况、关节功能恢复情况。结果研究组下肢深静脉血栓形成等并发症总发生率明显低于对照组(P<0.05),SDS和SAS评分明显低于对照组(P<0.05),FAM评分明显高于对照组(P<0.05),护理满意度明显高于对照组(P<0.05),生活质量评分明显高于对照组(P<0.05),骨折复位情况高于对照组(P<0.05),VAS评分明显低于对照组(P<0.05),关节功能恢复优良率明显高于对照组(P<0.05)。结论对髋部骨折使用围术期临床护理路径,有效降低下肢深静脉血栓形成等并发症发生率,预防下肢深静脉血栓形成,促进骨折复位,缓解疼痛,改善关节运动功能,减轻患者焦虑、忧郁等负面情绪,可推广应用。Objective To analyze the effect of the perioperative clinical nursing path for hip fractures on preventing deep vein thrombosis of the lower extremities.Methods A total of 52 hip fracture patients in our hospital from January 2018 to December 2019 who cooperated in this study were selected and divided into the study group and the control group according to a random number table,with 26 cases in each group.The study group received the perioperative clinical nursing path.The control group adopted routine nursing.The complications such as deep vein thrombosis of the lower limbs,self-rating depression scale(SDS)and self-rating anxiety scale(SAS),Fugl-Meyer assessment(FAM)and Visual Analogue Scale/Score(VAS)scores,and nursing satisfaction of the two groups were compared.The quality of life,fracture reduction,and joint function recovery of the two groups were evaluated.Results The total rate of complications such as deep vein thrombosis in the study group was lower than that in the control group(P<0.05).The scores of SDS and SAS in the study group were lower than those in the control group(P<0.05).The FAM score in the study group was higher than that in the control group(P<0.05).the nursing satisfaction in the study group was higher than that in the control group(P<0.05).The quality of life score in the study group was higher than that in the control group(P<0.05).The fracture reduction in the study group was higher than that in the control group(P<0.05).VAS score in the study group was lower than that in the control group(P<0.05).The excellent and good rate of joints functional recovery was higher than that in the control group(P<0.05).Conclusion The use of perioperative clinical nursing path for hip fractures can effectively reduce the rate of complications such as deep vein thrombosis of the lower extremities,prevent deep vein thrombosis of the lower extremities,promote fracture reduction,relieve pain,improve joint movement function,and reduce patient anxiety and depression.It can be promoted and applied.
