老年鼻饲患者居家照顾者压力现状及其影响因素研究

摘　　要：目的调查居家老年鼻饲照顾者压力现状,分析其影响因素,为构建老年鼻饲患者居家照顾者培训干预提供参考依据。方法选取2020年6月至2021年5月深圳市人民医院收治的老年鼻饲患者居家照顾者62名作为研究对象。采用一般资料调查表、照顾者压力量表对其进行调查,分析老年鼻饲患者居家照顾者压力。结果老年鼻饲患者居家照顾者压力处于一个较高的水平[(8.37±2.38)分],比标准常模6分高;分析其压力来源主要受教育程度和照顾者年龄的影响(R^(2)=0.375,调整后R^(2)=0.280,F=3.967,P=0.001)。结论老年鼻饲患者居家照顾者承受着较高的压力,受多种因素的影响,与疾病的直接后果有关。重视并了解影响居家照顾者的负担状况对于为老年人提供保健的医护人员来说非常重要。照顾老年人需要的是团队建设,将照顾者纳入医疗保健团队是至关重要的。医护人员采取个性化措施,以提高照顾者技能,减少压力,增强照顾者身心健康,从而促进患者的康复。Objective To investigate the stress status of home caregivers and analyze the influencing factors,so as to provide reference for training intervention of home caregivers.Methods A total of 62 home caregivers of elderly patients with nasal feeding from June 2020 to May 2021 in Shenzhen People′s Hospital were selected as the research subjects.The general data questionnaire and caregiver pressure scale were used to investigate the stress of home caregivers of elderly patients with nasal feeding.Results The stress of home caregivers of elderly patients with nasal feeding was at a high level of(8.37±2.38)points,which was higher than the standard norm(6 scores).The influences of education level and caregivers′age on stress sources were analyzed(R^(2)=0.375,adjusted R^(2)=0.280,F=3.967,P=0.001).Conclusion Home caregivers of elderly patients with nasal feeding suffer from high stress,which is influenced by many factors relating to the direct consequences of disease.It is important for health care providers providing care to elderly persons to appreciate and understand the burden conditions affecting home caregivers.Caring for elderly people requires team building,and integrating caregivers into the healthcare team is critical.The health of the patient is enhanced through individualized interventions by health care providers to improve caregiver skills,reduce stress and promote caregivers′physical and mental health,so as to promote patients′health.
