喉罩在小儿腹腔镜疝气手术中的应用效果观察

摘　　要：目的:观察喉罩在小儿腹腔镜疝气手术中的应用效果。方法:选取2020年3月—2021年3月临沭县人民医院收治的74例小儿腹腔镜疝气手术患儿,随机分为观察组和对照组各37例。观察组给予喉罩麻醉,对照组给予常规气管插管麻醉。比较两组不同时段SBP、DBP、HR指标差异,潮气量、PErCO_(2)与气道压力差异及拔管时间、麻醉药物剂量与复苏时间。结果:观察组患儿气管导管/插入喉罩后(T2)、拔管(T4)等时段SBP、DBP、HR指标均低于对照组,差异有统计学意义(P<0.05);观察组潮气量、PErCO_(2)与气道压力等指标均低于对照组,差异有统计学意义(P<0.05);观察组拔管时间、麻醉药物剂量与复苏时间等指标均优于对照组,差异有统计学意义(P<0.05)。结论:小儿疝气患儿在腹腔镜手术治疗时选取喉罩麻醉,可维持血压、心率稳定,减轻气道压力、减少潮气量,同时可减少麻醉用药,缩短复苏时间,安全可行。Objective To observe the efficacy of laryngeal mask for laparoscopic hernia in children.Methods From March 2020 to March 2021,74 cases of children in our hospital were randomly divided into the observation group(37 cases)under laryngeal mask anesthesia and the control group(37 cases)under conventional tracheal intubation anesthesia.The differences in SBP,DBP and HR indexes between the two groups at different time periods were analyzed.Differences in tidal volume,PErCO_(2) and airway pressure,extubation time,anesthetic dose and resuscitation time.Results SBP,DBP and HR indexes in the observation group were lower than those in the control group at T2 and T4 after tracheal tube/laryngeal mask insertion,(P<0.05);The indexes of tidal volume,PErCO_(2) and airway pressure in the observation group were lower than those in the control group(P<0.05);The extubation time,anesthetic dose and resuscitation time in observation group were better than those in control group(P<0.05).Conclusions Laryngeal mask anesthesia during laparoscopic surgery for children with hernia can maintain stable blood pressure and heart rate,reduce airway pressure,reduce tidal volume,and reduce anesthetic medication and resuscitation time,which is safe and feasible.
