公立与民营口腔医疗机构种植牙费用的比较研究:基于上海市典型案例分析

摘　　要：目的:比较上海市公立与民营机构种植牙医疗费用,分析种植牙价格形成的原因及对策。方法:采用典型案例法,选择上海市3家公立医院和3家民营医院,对医务人员进行个人深度访谈。结果:种植牙的平均医疗费用约为3万元,民营医院高端种植品牌的总费用水平高出公立医院约30%。两类机构的成本结构存在明显差异,民营医院的采购成本显著低于公立医院,但人力成本和场地租金等更高。以进口为主的市场供给模式、公立医院控制成本的动力不足、以及信息不对称下医患双方对高价产品的偏好,是导致种植牙高费用的主要原因。结论:患者接受种植牙服务的经济负担较重;公立与民营机构的成本结构存在明显差异。建议从促进市场竞争、加强流通监管、改革付费方式、提高信息共享等方面进一步完善种植牙耗材管理。Objective:To compare the medical expenses for dental implants in public and private institutions in Shanghai,analyze the reasons and countermeasures for the formation of implant prices.Methods:Typical case study and personal in-depth interviews are conducted with medical staffs in 3 public hospitals and 3 private hospitals in Shanghai.Results:Generally,the average total expense of dental implants is about 30 000 yuan.The total cost of high-end planting brands in private hospitals is about 30% higher than that of public hospitals.There are obvious differences in the cost structure of public and private institutions.The private institutions have much lower procurement cost but higher labor and site rents costs.The import-based market supply model,the lack of strong willingness of cost control in public hospitals,and the preference of doctors and patients for high-priced products under asymmetric information are the main reasons for the high cost of dental implants.Conclusion:The economic burden for patients taking dental implant is relatively heavy.There are obvious differences in the cost structure of public and private institutions.It can strengthen the price governance of dental implants in the future from promoting market competition;regulating the supervision of the production and circulation of implant consumables;reforming the payment methods and strengthening the information sharing.
