逆境商及乐商在老年慢性心力衰竭患者心理弹性与自我管理之间中介效应分析

摘　　要：目的分析老年慢性心力衰竭(chronic heart failure,CHF)患者逆境商、乐商、心理弹性以及自我管理现状及相关性,为老年CHF患者心理干预及提升其自我管理提出建议。方法选取哈尔滨医科大学附属第一医院2018年1月—2019年1月间住院治疗的475例老年CHF患者作为研究对象,调查其逆境商、乐商在心理弹性和自我管理水平的相关性,建立结构方程模型分析中介效应。结果老年CHF患者心理弹性得分为(30.49±8.54)分、自我管理得分为(54.29±6.35)分、逆境商得分为(119.25±19.06)分、乐商得分为(62.34±12.63)分。逆境商及乐商在老年CHF患者心理弹性与自我管理之间中介效应分别为0.18和0.17,逆境商及乐商链式中介效应为0.08。结论老年CHF患者高逆境商、高乐商能提高其心理弹性、自我管理水平,在心理弹性与自我管理中起中介效应和链式中介效应,可采取给予老年CHF患者专业心理辅导,提升其逆境商和乐商水平,从而增强其自我管理能力。Objective To analyze the current status and correlation of adversity quotient,music quotient,psychological resilience and self-management in elderly patients with chronic heart failure in order to providing suggestions for psychological intervention of elderly patients with chronic heart failure and improving their self-management.Methods 475 elderly patients with chronic heart failure who were hospitalized to the First Affiliated Hospital of Harbin Medical University between January 2018 and January 2019 were selected as the research objects.Their psychological resilience,self-management level,adversity quotient and optimistic intelligence quotient were investigated.The structural equation model was established to analyze the mediating effect.Results The score of psychological flexibility was(30.49±8.54),the score of self-management was(54.29±6.35),the score of adversity quotient was(119.25±19.06)and the score of optimistic intelligence quotient was(62.34±12.63)in the elderly patients with chronic heart failure.Adversity quotient and optimistic intelligence quotient played an intermediary effect in the psychological elasticity and self-management of elderly patients with chronic heart failure(P<0.05).The intermediary effects were 0.18 and 0.17,and their continuous path of the two had a chained mediation effect at 0.08.Conclusion Elderly chronic heart failure patients with high adversity quotient and high optimistic intelligence quotient can improve their psychological resilience and self-management level,which play an intermediary effect and chain interme-diary effect in psychological resilience and self-management.It can take professional psychological counseling for patients with chronic heart failure to improve their adversity quotient and music quotient level,so as to enhance their self-management ability.
