基于卫生资源供给结构视角对新型农村合作医疗制度受益公平性的研究

摘　　要：目的基于卫生资源供给机制差异,对比分析福建省新农合制度下不同卫生资源水平的居民受益公平性和影响因素。方法运用聚类分析方法,提取卫生资源配置水平的特征区间,利用固定效应模型分析卫生资源配置对新农合制度受益和补偿水平的影响。结果福建省卫生资源配置水平可划分为高、中、低三大区间,回归结果显示,地区经济发展水平对新农合受益公平性有正向作用,但卫生资源配置情况与受益水平呈负向变化关系,千人均床位、卫生技术人员的规模越大,居民实际获得的补偿水平越低,即不同卫生资源配置区间新农合实际补偿率有差别。结论卫生资源配置会影响医疗保障制度的受益公平,因此,必须优化卫生资源配置结构,同时继续推进城乡居民医保制度整合,保障居民更加公平地享有基本医疗服务,从而改善医疗保障制度的受益公平性。Objective To compare and analyze the benefit equity and influencing factors of residents with different health resource levels under the new rural cooperative medical system based on the differences in health resource supply mechanisms.Methods Using the cluster analysis method,the characteristic interval of the allocation level of health resources was extracted.The fixed effect model was established to analyze the impact of health resource allocation on the benefit and compensation level of the new rural cooperative medical system.Results The allocation le-vel of health resources allocation was divided into three sections:high,medium and low.The regression showed that the regional economic development level had a positive effect on the equity of the new rural cooperative medical system,but the allocation of health resources and the level of equity were negative.The larger scale of beds and health technicians per 1000 was,the lower the compensation level actually received by residents.The actual compensation rate of the new rural cooperative medical system varied in different health resource allocation intervals.Conclusion The allocation of health resources affects the benefit equity of the medical security system.Therefore,it is necessary to optimize the allocation structure of health resources,and continue to promote the integration of the medical insu-rance system for urban and rural residents,so as to guarantee residents more equitable access to basic medical ser-vices and improve the fairness of the medical security system.
