布比卡因+亚甲蓝混合液肛周封闭联合耳穴埋籽对混合痔患者术后疼痛及水肿的影响

摘　　要：目的:探讨布比卡因+亚甲蓝混合液肛周封闭联合耳穴埋籽对混合痔术后患者的影响。方法:选取2019年2月-2020年11月邳州市中医院确诊为混合痔并进行手术的102例患者。以患者自愿选择治疗方式为原则,将其分为对照组与观察组,每组51例。两组均行外剥内扎术,对照组术后采用亚甲蓝+灭菌注射用水混合液肛周封闭治疗,观察组术后则采用布比卡因+亚甲蓝混合液肛周封闭联合耳穴埋籽治疗。比较两组术前,术后6、24 h,3、5、7 d疼痛程度,术后6、24 h,3、5、7 d水肿程度及术后不良反应。结果:术前两组疼痛程度评分比较差异无统计学意义(P>0.05),观察组术后6、24 h,3、5、7 d疼痛程度评分和水肿程度评分均低于对照组(P<0.05)。术后观察组不良反应6例(焦虑1例,心律失常1例,食欲减退1例,排便困难2例,睡眠障碍1例),对照组术后不良反应18例(焦虑2例,心律失常4例,食欲减退1例,排便困难7例,睡眠障碍4例),观察组术后不良反应发生率低于对照组(P<0.05)。结论:布比卡因+亚甲蓝混合液肛周封闭联合耳穴埋籽用于混合痔患者术后的治疗中,与单一的治疗方式比较,前者止痛和消肿效果更加明显,术后的不良反应发生率低,是临床上值得推广采用的混合痔术后联合治疗手段。Objective:To investigate the effect of perianal sealing with Bupivacaine + Methylthioninium mixed solution combined with ear point seed embedding on patients with mixed hemorrhoids after surgery.Method:A total of 102 patients with mixed hemorrhoids diagnosed and operated in Pizhou Hospital of Traditional Chinese Medicine from February 2019 to November 2020 were selected.Based on the principle of patients’ voluntary choice of treatment method,they were divided into control group and observation group,with 51 cases in each group.Both groups were treated with external stripping and internal ligation.After surgery,the control group was treated with perianal sealing with Methylthioninium + sterilized water for injection mixed solution,while the observation group was treated with perianal sealing with Bupivacaine + Methylthioninium mixed solution combined with ear point seed embedding.The degree of pain before surgery,at 6,24 h,3,5 and 7 d after surgery,the degree of edema at 6,24 h,3,5 and 7 d after surgery and postoperative adverse reactions were compared between the two groups.Result:There was no difference in pain degree between the two groups before surgery (P>0.05),the scores of pain degree and edema degree in observation group were lower than those in the control group at 6,24 h,3,5 and 7 d after surgery (P<0.05).There were 6 cases of postoperative adverse reactions in the observation group (1 case of anxiety,1 case of arrhythmia,1 case of anorexia,2 cases of difficult defecation and 1 case of sleep disorder),and 18 cases of postoperative adverse reactions in the control group (2 cases of anxiety,4 cases of arrhythmia,1 case of anorexia,7 cases of difficult defecation and 4 cases of sleep disorder).The incidence of postoperative adverse reactions in the observation group was lower than that in the control group (P<0.05).Conclusion:In the postperative treatment of patients with mixed hemorrhoids,the perianal sealing with Bupivacaine + Methylthioninium mixed solution combined with ear point seed embedding has mo
