摘 要:目的:分析胰岛素样生长因子-b(IGF-b)、白介素-17(IL-17)、降钙素原(PCT)联合检测对患儿支气管肺炎的诊断价值。方法:选择宜宾市第二人民医院2019年1月-2021年1月诊治的支气管肺炎患儿200例,将其纳入研究组。另在同期选择健康体检儿童200例,将其纳入对照组。抽取两组肘静脉血5 ml,低速离心处理后(3 000 r/min)留取血清,使用放射免疫分析法检测IGF-b水平,使用酶联免疫吸附法检测IL-17水平,使用电化学发光法检测PCT水平。比较两组IGF-b、IL-17、PCT水平和不同病情严重程度IGF-b、IL-17、PCT水平;采用受试者工作特征(ROC)曲线分析IGF-b、IL-17、PCT单独及联合检测诊断支气管肺炎的临床价值。结果:研究组IGF-b(0.98±0.12)μg/L,IL-17(125.13±33.41)pg/ml,PCT(10.58±1.25)ng/ml,明显高于对照组(P<0.05)。重度患儿IGF-b、IL-17、PCT水平均明显高于轻度患儿(P<0.05),轻度患儿IGF-b、IL-17、PCT水平明显高于对照组(P<0.05)。IGF-b、IL-17、PCT联合诊断的曲线下面积(AUC)大于IGF-b、IL-17、PCT单独检测(χ^(2)=6.164、7.128、4.254,P<0.05)。结论:IGF-b、IL-17、PCT在支气管肺炎患儿中高表达,且病情越严重的患儿其表达水平越高,联合三项检查有利于提升支气管肺炎诊断价值。Objective:To analyze the diagnostic value of combined detection of insulin-like growth factor-b (IGF-b),interleukin-17 (IL-17) and procalcitonin (PCT) in children with bronchopneumonia.Method:A total of 200 children with bronchopneumonia diagnosed and treated in the Second People’s Hospital of Yibin City from January 2019 to January 2021 were selected and included in the study group.In addition,200 healthy check-up children were selected in the same period and included in the control group.In the two groups,5 ml of elbow vein blood of children were extracted,and the serum was left after low-speed centrifugation (3 000 r/min).The level of IGF-b was detected by radioimmunoassay,IL-17 was detected by enzyme-linked immunosorbent assay,and the level of PCT was detected by electrochemiluminescence.The levels of IGF-b,IL-17 and PCT between the two groups and the levels of IGF-b,IL-17,PCT in different disease severity were compared.The receiver operating characteristic (ROC) curve was used to analyze the clinical value of single and combined detection of IGF-b,IL-17 and PCT in the diagnosis of bronchopneumonia.Result:IGF-b of (0.98±0.12) μg/L,IL-17 of (125.13±33.41) pg/ml and PCT of (10.58±1.25) ng/ml in the study group were significantly higher than those in the control group (P<0.05).The levels of IGF-b,IL-17 and PCT in severe children were significantly higher than those in mild children (P<0.05),and the levels of IGF-b,IL-17 and PCT in mild children were significantly higher than those in the control group (P<0.05).The area under the curve (AUC) of the combined diagnosis of IGF-b,IL-17 and PCT was larger than that of IGF-b,IL-17 and PCT alone (χ^(2)=6.164,7.128,4.254,P<0.05).Conclusion:IGF-b,IL-17 and PCT are highly expressed in children with bronchopneumonia,and the more serious the condition is,the higher the expression level.The combination of the three tests is helpful to improve the diagnostic value of bronchopneumonia.
