王祥麒基于“五脏一体观”辨治原发性食管癌思路浅析

摘　　要：食管癌患者初期症状不明显,确诊时多是中晚期,失去了手术根治机会。放发疗又因副作用大而不耐受率高,缺乏长期有效的治疗手段。该文总结王祥麒教授基于“五脏一体观”辨治食管癌临证思路,认为五脏气机失调,痰、气、瘀、毒搏结于脘管,生坏肉恶疮,噎阻食道是食管癌的基本病机,强调五脏气机失调为病本,痰气瘀毒为病标,并从“脾虚气逆,肝郁气结,肾虚精亏,心肺不足”角度阐释其五脏失调病机。治疗策略上标本兼顾,调养五脏为主,以治其本,祛邪攻瘤,兼治病标,并总结王祥麒教授选方用药规律及特色。以期为食管癌患者提供一条可行、有效的中医辨治新思路。The initial symptoms of patients with esophageal cancer are not obvious,and most of them are in the middle and late stage,so they lose the opportunity of radical surgery.Radiation therapy has high intolerance rate due to large side effects,and Western medicine lacks long-term effective treatment.This paper summarizes Professor WANG Xiangqi’s clinical thinking of syndrome differentiation and treatment of esophageal cancer based on the“view of five Zang organs in one”,and holds that the dysfunction of five Zang organs,phlegm,Qi,blood stasis and poison pulse knot in the epigastric canal,bad meat and sores,and choking obstruction of the esophagus are the basic pathogenesis of esophageal cancer.It emphasizes that the imbalance of five Zang Qi mechanism is the root of the disease,phlegm,Qi,blood stasis and toxin is the symptom,and from“spleen deficiency and Qi inversion,liver depression and Qi knot,kidney deficiency and essence deficiency,heart and lung deficiency”Explain the pathogenesis of five Zang organs disorder from the perspective of.The treatment strategy is based on both the superscript and the original,focusing on the rehabilitation of the five internal organs,focusing on the treatment of the root,dispelling evil and attacking tumors,and also treating the symptoms.It also expounds the law and characteristics of Professor WANG Xiangqi’s prescription selection and medication.In order to provide a feasible and effective new idea of syndrome different iation and treatment for patients with esophageal cancer.
